High-resolution MR lymphangiography for planning lymphaticovenous anastomosis treatment: a single-centre experience

Abstract: MRL is easy and safe to use and combines extensive information on the anatomy and functionality of lymphatic vessels and veins in a single process; therefore, it could be useful in LVA treatment planning and evaluating possible super-microsurgical treatment complications in patients with lymphedema.

“…Despite many articles reporting on LVA to date, preoperative examinations vary by surgeon preferences and may include ICG lymphography, ultrasound, lymphoscintigraphy, or magnetic resonance imaging (Garza & Chang, 2018; Hara & Mihara, 2017; Hara & Mihara, 2019a; Hara & Mihara, 2019b; Koshima et al, 2000; Mazzei et al, 2017; Mihara, Hara, & Kawakami, 2018; Mihara, Hara, Kawakami, Zhou, et al, 2018; Phillips, Gore, Ramsden, & Furniss, 2019). In the current study, we applied multi‐lymphosome ICG lymphography to detect the lymphatic vessels.…”

“…Preoperative examinations for LVA have recently made progresses, making it possible to identify functional lymphatic vessels in cases of advanced‐stage lymphedema (Hara & Mihara, 2017; Hara & Mihara, 2019a; Mazzei et al, 2017; Mihara et al, 2018; Mihara, Hara, & Kawakami, 2018). In the current study, we evaluated the surgical results of LVA in advanced‐stage lymphedema patients (lymphoscintigraphic type 4 and 5).…”

“…As previously discussed, the sensitivity of breast MRI for the detection of breast cancer is high, but its specificity is only moderate, ranging from 37% to 100% [ 16 ]. It is then essential to biopsy suspicious MR-detected lesions to make a definitive diagnosis [ 22 ]; MR-guided breast biopsy is gradually increasing, especially when lesions are visible on MRI but not on conventional imaging [ 41 43 ]. Nevertheless, these techniques are not widely available and require the use of expensive MR magnets, time, and personnel [ 21 ].…”

“…MRI should be performed for characterizing a thymic lesion only in selected cases, such as the distinction between a small thymic neoplasm and an atypical thymic hyperplasia, exploiting dual gradient-echo in-phase and opposed-phase sequences. MR lymphangiography may be used in the suspicion of a lesion of lymphatic origin (69)(70)(71)(72)(73).…”
