High prevalence of undiagnosed patients with peripheral arterial disease in patients hospitalised for non-vascular disorders

Abstract: In a tertiary care hospital, the prevalence of unrecognised PAD among patients hospitalised for non-PAD-related causes is high and the preventive CVD therapy rates are low. Hospitalisation is a good opportunity to detect PAD.

“…1,3,16,24,[29][30][31][32] The German getABI study, for example, identified older age, diabetes, hypertension, lipid disorders, coexisting atherothrombotic disease, and previous cerebrovascular or cardiovascular events as risk factors. 29 In the United States NHANES study, black race, smoking, diabetes, hypertension, hypercholesterolemia, and poor kidney function were associated with PAD; 32 and at least one cardiovascular disease risk factor was identified in 95% of patients.…”

“…Previous studies, which found lower PAD rates (ϳ30%), were performed in lower risk patients such as patients hospitalized for nonvascular diseases and cardiovascular outpatients followed by general practitioners. 1,24 In the Swiss Atherothrombosis Survey, the occurrence of PAD was assessed in everyday Swiss practice using ABI. The study revealed that 3.7% of patients had symptomatic PAD and an additional 2.7% with undiagnosed PAD.…”

“…A 36% prevalence of low ABI has been described when ABI was measured by pulse palpation [12]. Hand held Doppler measurements recently showed a 29 % PAD prevalence [13], similar to that observed among patients hospitalised for acute coronary and cerebrovascular events [14,15], and in those hospitalised for non-vascular causes in a tertiary care hospital [16].…”

“…7 In addition, a high prevalence of unrecognized PAD in patients hospitalized for nonvascular diseases has been reported. 8 As atherosclerosis is a systemic process, PAD often coexists with vascular disease in other arterial beds (eg, the coronary or cerebrovascular systems).…”
