High magnification chromoscopic colonoscopy or high frequency 20 MHz mini probe endoscopic ultrasound staging for early colorectal neoplasia: a comparative prospective analysis

Abstract: Background: Successful endoscopic management of early colorectal cancer using endoscopic mucosal resection requires the mandatory prediction of invasive depth and lymph node metastasis. Previous data using the Nagata crypt types Vn(B)/(C) as clinical indicators of T2/N+ disease have shown low specificity (50%) with a tendency to over stage lesions. New mini probe ultrasound ''through the scope'' imaging permits staging of lesions proximal to the rectum using direct endoscopic visualisation. Aim: To compare the… Show more

“…Our attempt to examine the large lesion resulted in understaging of the pT3 as uTis in one case. As reported in the literature, Hurlstone et al [19] achieved acoustic coupling for 100% of their patients. Nevertheless, they included only nonpolypoid lesions and used the balloon-assistance technique in 4 (7%) of the 52 cases.…”

“…Vertical penetration to 1000 µm (the level of the muscularis propria) might be a powerful clinical tool as in vivo management could be modified further without the requirement for a second high-frequency endoscopic ultrasonographic procedure to delineate disease stage and hence suitability for definitive endoscopic resection or diagnostic biopsy 3,22 .…”

“…A previous study showed that endoscopic ultrasonography had higher diagnostic accuracy than pit patterns based on magnifying endoscopy in T staging [18], whereas another study found that the diagnostic accuracy of those methods was similar [19]. Recently, Mukae et al [20] found that the sensitivity and specificity of endoscopic ultrasonography in estimating the deep invasion depth of early CRC was 90 and 87%, respectively.…”
