Endoscopic Approach for Superficial Colorectal Neoplasms

Abstract: Background: Colorectal cancer (CRC) is the third most commonly diagnosed cancer in males and the second in females, with an estimated 1.4 million cases and 693,900 deaths in 2012. Colonoscopy is the cornerstone for the detection and prevention of CRC. In addition, endoscopic treatment for CRC at an early stage can effectively improve patients' quality of life and cure rate. Summary: This review focuses on endoscopic approaches, including white light endoscopy, chromoendoscopy, magnifying endoscopy and therapeu… Show more

“…However, this makes it difficult to obtain a precise histopathological diagnosis. Also incomplete resection and local recurrence rates are high in piecemeal EMR [1-4]. Therefore, EMR and piecemeal EMR are unsuitable for the treatment of colorectal neoplasms that is larger than 20 mm in diameter, including LSTs.…”

“…Endoscopic mucosal resection (EMR) is widely accepted and regularly used for treatment of precancerous and early colorectal neoplasms. However, this technique is not feasible and safe for en bloc resection if the tumor size is larger than 20 mm, as it may not facilitate accurate histologic diagnosis and reduced recurrence rates [1-4].…”

“…This process is called 'shared decision making'. 9 A recent systematic review assessing the quality of such DAs for various diseases concluded that patients using DAs have (1) a better knowledge of treatments, (2) are better informed about treatments and (3) have a better understanding of their personal values compared to patients in usual care. 10 This seems promising, but patients also increasingly use the internet as an important source of health information 11 and the DAs found through the web are not included in such large-scale systematic reviews.…”

“…Endoscopic submucosal dissection (ESD) and endoscopic mucosal resection (EMR) are effective techniques for removal of early neoplasia of the gastrointestinal tract by separating submucosa and muscular layer [1][2][3][4]. In some situations, such as submucosal fibrosis, previous failed resection, or submucosal invasion, these techniques may not achieve en bloc and histologically complete (R0) resection [5][6][7][8]. Endoscopic full-thickness resection could be a valid alternative to conventional surgery.…”
