Objective: To identify rationale for removal of the first Hickman catheter implanted in patients undergoing allogeneic hematopoietic stem cell transplantation, the micro-organisms involved in the occurrence of infection, and the length of time the catheter was in situ. Methods: A cross sectional, retrospective study was conducted. The sample consisted of 57 transplant recipients. To conduct chart review, an instrument was developed containing variables related to patient identification, time of catheter use, reason for withdrawal, and isolated micro-organisms. Results: Among the reasons for catheter removal, frequent infection (49%) was the most common; the Stenotrophomonas maltophilia microorganism (25%) was the most frequently isolated. Conclusions: Due to the high incidence of infectious complications leading to Hickman catheter removal, it is essential to standardize catheter care for the health care team, patients and their caregivers. Métodos: Estudo transversal retrospectivo. A amostra foi constituída por 57 prontuários de pacientes transplantados. Para a obtenção dos dados, elaborou-se um instrumento contendo variáveis relativas à identificação do paciente, tempo de permanência do cateter, motivo de retirada e micro-organismo isolado. Resultados: Dentre os motivos de retirada do cateter, destacou-se como o mais frequente a infecção (49%). O Stenotrophomonas maltophilia (25%) foi o micro-organismo identificado com maior frequência. Conclusões: Diante da elevada incidência de complicações infecciosas que levam à retirada do cateter de Hickman, faz-se necessária uma padronização de cuidados relacionados a esse cateter, tanto para a equipe de saúde como ao paciente e seu cuidador. Dressing is an intervention aimed to prevent infection in central venous catheter. This study aimed to analyze the frequency of catheter-related infection and skin toxicity in the use of transparent film in Hickman's catheter in patients who underwent allogeneic hematopoietic stem cell transplantation. A case series with 10 cases was carried out. Due to the presence of exudate on the average for 12 days, sterile gauze dressing was used for 12.9 days (average). Transparent film was used, on average, for 15.1 days. Catheters were precociously removed due to infection in four cases. The highest degree of skin toxicity occurred in a case that used gauze dressing and in three cases with film. The transparent film permitted visualization of the exit site of the catheter and changes with longer intervals. Descriptors Uso de la película transparente de poliuretano en el catéter venoso central de larga permanenciaEl curativo es una intervención que tiene por objetivo la prevención de infección en el catéter venoso central. El estudio tuvo como objetivo analizar la frecuencia de infección relacionada al catéter y la toxicidad cutánea en la utilización del curativo de poliuretano en el catéter de Hickman implantado en pacientes sometidos al trasplante de células tronco hematopoyéticas alogénicas. Para esto se realizó una serie de 10 casos. El exudado estuvo presente en promedio por 12 días, siendo necesario el uso del curativo de gasa estéril con cinta adhesiva por 12,9 días (promedio). El curativo de poliuretano fue utilizado en promedio por 15,1 días. La retirada precoz del catéter debido a surgimiento de infección ocurrió en cuatro casos. El mayor grado de toxicidad cutánea ocurrió en un caso que se utilizó el curativo de gasa y en tres casos que utilizaron la película. La película transparente permitió la visualización del sitio de salida del catéter y la realización del cambio en intervalos mayores.

Bromhidrosis is a clinical disorder characterized by excessive or abnormal foul axillary odour due to the interaction of apocrine glands with micro-organisms which causes a serious personal and social handicap for affected people. We present the case of a 50-year-old caucasian female with bromhidrosis. The patient referred that this symptom had begun two months previously. Her past treatments included antibacterial soap, topical antibacterial agents and perfumes, but none of these relieved the patient of the odour. A cultural examination of axillary smear was carried out and it revealed the presence of ciproftoxacin sensible Sphingomonas paucimobilis. Therefore the patient was treated with ciproftoxacin and after 1 week the infection resolved completely.Bromhidrosis is a common but unpleasant disease faced by many populations, particularly in Asia, due to the interaction of apocrine glands with microorganisms. Axillary bacteria degrade the apocrine sweat and malodorous smell develops within a few hours. The major odour components are short-chain fatty acids and ammonia (1). However, the definite etiology ofbromhidrosis has not yet been generalized. To our knowledge, this is the first report of axillary bromhidrosis caused by Sphingomonas paucimobilis. MATERIALS AND METHODSA 50-year-old caucasian female was referred to our Department in May 2008, because of a disagreeable odour due to axillary bromhidrosis. Past medical history was unremarkable; the dermatological examination showed no abnormalities and there were no superficial axillary lymphadenopathy; laboratory tests were normal. The patient referred that this symptom had begun two months previously. Her past treatments had included antibacterial soap, topical antibacterial agents, and perfumes, but none of these relieved the patient of the odour. An axillary smear was performed and processed according to standard microbiologic techniques in the Microbiology laboratory of "Umberto I" Hospital in Rome. In particular, the specimen was cultured on Sabouraud agar for yeasts and on routinely selective and elective culture media for bacteria and it was incubated in ambient air at 37°C for 24-48 hours.
