Hepatocellular Carcinoma Occurrence/Recurrence after Direct-Acting Antivirals for Hepatitis C in Egyptian Cohort: Single-Center Experience

Abstract: Background: Conflicting data have been published about the risk of hepatocellular carcinoma (HCC) following direct-acting antivirals (DAAs). We investigated the incidence of HCC occurrence/recurrence after DAAs therapy. Patients and Methods: Retrospectively, we analyzed data of 392 patients with F3–4 fibrosis and cirrhosis treated by DAAs during the period from August 2015 to May 2018. In HCC-experienced patients, HCC treatment modality, and the duration between HCC management and DAAs initiation were recorded… Show more

“…An early meta-analysis showed high HCC recurrence rates of 7.4% after six months and 47.0% after two years in untreated patients after an initial curative resection or ablation of HCC, but also extreme variability in recurrence risk and survival [18] . An uncontrolled, real-world Spanish retrospective study confirmed the initial high HCC recurrence rates [19] . Similar unexpectedly high occurrence and recurrence rates were reported in more uncontrolled prospective and retrospective studies [20][21][22][23][24][25] .…”

“…This observation needs post‐treatment ultrasound and FibroScan . Moreover, controversial data have emerged on HCC occurrence and recurrence after HCV eradication with IFN‐free treatment regimens . HCC showed to be correlated with lowered serum albumin levels …”

“…Another study denied the occurrence of HCC after DAAs although a high incidence of recurrence was still found. This study also suggested that high AFP before treatment is a good predictor for developing HCC[ 99 ].…”
