Health effects of intermittent fasting: hormesis or harm? A systematic review

Abstract: Clinical research studies of fasting with robust designs and high levels of clinical evidence are sparse in the literature. Whereas the few randomized controlled trials and observational clinical outcomes studies support the existence of a health benefit from fasting, substantial further research in humans is needed before the use of fasting as a health intervention can be recommended.

“…During the last decade the health‐promoting and life‐extending effects of a wide variety of variants of a standard CR diet have been examined in both humans and models species 29, 57, 58, 59, 60, 61, 62. The major differences between the dietary variants of intermittent calorie restriction are (1) the extent of calorie reduction varying from complete fasting to a mild decrease in calorie intake and (2) the time window when the fasting/CR is applied in combination with the period in between the fasting/CR cycles.…”

“…While long-term maintenance of caloric restriction is challenging, intermittent fasting appears to provide a competitive alternative that is more realistic to implement [3]. Intermittent fasting regimens can produce benefits with fasting periods as brief as 16 –24 hours [18;19;27]. Preclinical and clinical studies have reported multiple adaptive changes following intermittent fasting [28].…”

“…Although clinical trials implementing various schedules of interment fasting are few, the findings and adherence thus far have been positive [18]. Horne and colleagues investigated acute biological changes in response to a one-day, water only fast in healthy adults (ages 19 to 64) [19].…”
