Guttural pouch leiomyosarcoma causing nasopharyngeal compression in a pony

Abstract: Summary A 14‐year‐old Connemara cross gelding presented with abnormal respiratory noise and exercise intolerance. Upper airway endoscopy, ultrasonography, radiography and computed tomography revealed a large mass within the left guttural pouch causing marked left dorsal nasopharyngeal collapse and displacement and compression of the right guttural pouch. The horse was subjected to euthanasia and a post‐mortem examination confirmed the above findings. Histological and immunohistochemical examinations of the mas… Show more

“…IHC is an important diagnostic tool in determining the cellular origin of neoplasms because of its practicality, high sensitivity, and specificity when compared to other diagnostic methods (WILSON, 2017). In the evaluated sections, there was moderate and diffuse cytoplasmic reactivity in the neoplastic cells for the antibodies against desmin, 1A4, and HHF35, thus confirming the muscular origin and specifying the smooth muscle type using markers 1A4 and HHF35, given that the latter markers are specific for alpha-smooth muscle actin (DREW et al, 2016;GRACCHI et al, 2020).…”

“…The larynx and other perioesophageal tissues also cannot be excluded as the site of origin; however, this would point towards the presence of a leiomyosarcoma with local invasion rather than a benign leiomyoma. A leiomyosarcoma has recently been reported within the caudal aspect of the guttural pouch (Drew et al, 2018).…”

“…Hematomas can occur in some cases of trauma, especially to adjacent musculature. Neoplasia involving the guttural pouch is uncommon but cases of fibroma, melanoma, squamous cell carcinoma, hemangiosarcoma and leiomyosarcoma have been described [ 5 , 13 , 21 ]. A case of a large fungal granuloma secondary to guttural pouch mycosis was also reported [ 22 ].…”

“…Diagnostic imaging is particularly useful for evaluation of large masses obliterating the pouches because endoscopy may not be possible in these cases and diagnostic imaging provides a better evaluation of the structures surrounding the pouches. Radiographic examination may reveal an increased soft tissue opacity obliterating the air-filled guttural pouch, with a mass effect displacing its contours [ 5 , 21 ]. The contours of the pouch can be compressed if the mass is originating from surrounding structures, or expanded if the mass is located within the pouches [ 5 ].…”
