IntroductionIt is inaccurate to assess blood glucose with glycated hemoglobin (HbA1c) in patients with diabetes and chronic kidney disease (CKD), and whether glycated albumin (GA) is better than HbA1c in these patients remains unclear.MethodsWe searched PubMed, Embase, Web of Science, Scopus, the Cochrane Library, and MEDLINE to July 2017 for studies that investigated the correlation between GA or HbA1c and the average glucose levels (AG) relevant to this theme. Statistical analysis was performed using RevMan5.3 and Stata12.0. The outcome was the correlation coefficient between GA or HbA1c and AG. For the first time, we made a comparison of GA and HbA1c in different CKD stages.ResultsA total of 24 studies with 3928 patients were included. Early stages of CKD refer to CKD stage 1 to 3. Advanced CKD refer to CKD stage 4 and 5 including patients receiving dialysis. The meta-analysis suggested that in early stages of CKD, the pooled R between GA and AG was 0.61 (95% CI = 0.49−0.73) and 0.71 (95% CI = 0.55−0.87) for HbA1c (P > 0.05). In advanced CKD patients, the pooled R between GA and AG was 0.57 (95% CI = 0.52−0.62), and 0.49 (95% CI = 0.45−0.52) for HbA1c (P = 0.0001).ConclusionGA is superior to HbA1c in assessing blood glucose control in diabetes patients with advanced CKD.

Prediabetes increased risk of diabetes and diabetes-related macrovascular and microvascular complication. Glycohemoglobin (HbA 1c ) is clinically used as the gold standard for glycemic control of diabetes. Glycated albumin (GA) is an early Amadori-type glycation protein between glucose and serum albumin, which changes in a shorter period of time than HbA 1c and is superior to HbA 1c in reflecting fluctuations in blood glucose. In this study, we aim to assess the predictive value of GA and glycohemoglobin (HbA1c) on a progression of diabetic retinopathy (DR) in prediabetic patients in Taiwan. This study was conducted at the outpatient department of a regional hospital in Southern Taiwan, and recruited 291 patients with prediabetes from January 2016 to February 2017. Blood and urine samples were obtained from all patients after fasting for 12 hours within 1 month of enrollment. The mean age is 62.5 ± 13.0 years and there are 161 males and 130 females. A total of 24.1% of patients have DR. The average value of GA and HbA1c are 14.6% ± 2.8% and 6.0% ± 0.4%, respectively. Old age, male, high systolic blood pressure, high HbA 1C , and low total cholesterol are significantly associated with DR in patients with pre-DM. Therefore, in the prediabetic populations, high HbA 1C, but not GA nor GA/HbA 1C ratio, is significantly associated with DR. K E Y W O R D Sglycated albumin, HbA1c, prediabetes, retinopathy

Diabetes mellitus (DM) is one of the world's health problem with increasing prevalence and important medical and social impacts. Diabetes mellitus causes micro and macrovascular complications depending on the presence of blood glucose level disorder. Impaired glucose levels have two components, chronic hyperglycemia and acute fluctuations of blood glucose levels from peak to nadir. These two components cause DM with two main mechanisms, protein glycation, and oxidative stress. Glycated albumin describes uncontrolled medium range glucose level while malondialdehyde reflects oxidative stress. Nephrin is one of the damage markers of podocyte that represents diabetic kidney disease. To find out the correlation between uncontrolled blood glucose with urinary nephrin level in a patient with type 2 diabetes mellitus. This is an observational study with the cross-sectional method. Subjects are 30 types 2 DM patients. Patients serum glycated albumin (GA) are measured, along with serum malondialdehyde (MDA) and urinary nephrin. Among 30 types 2 DM patients, there is a significant increase in GA and MDA level. Mean serum GA level is 20,87% (5,91) %, serum MDA level is 9,24 (5,59-13,59) and urinary nephrin is 369,25 (6-3952) ng/ml. There is a moderate correlation between serum GA with urinary nephrin (p=0,027) and moderate correlation between serum MDA and urinary nephrin (p=0,003). There is a correlation between uncontrolled blood glucose with urinary nephrin level in type 2 diabetes mellitus.
