Syphilis is a disease that is found all over the world that causes damaging effects to the fetus through vertical transmission. This study aimed to analyze the processes that trigger the vertical transmission of syphilis through gestational and congenital syphilis notifications. It is a cross-sectional study. The sample totaled 129 notifications of syphilis in pregnant women and 132 notifications of congenital syphilis in the city of Natal, from 2011 to 2015. Data were obtained from the Information System for Disease Notification. The Chi-square, Student’s and Fisher’s tests were used to verify associations of interest. Diagnosis of maternal syphilis was predominant in the third trimester of pregnancy. Only 1.6% of the pregnant women were registered with an adequate treatment regimen, of these 16.3% had the concomitant treatment with their partners. Of the affected children, 78.8% were registered as asymptomatic. The factors that trigger vertical transmission are related to the late diagnosis of the pregnant woman and sexual partner(s) and the deficiencies in clinical/therapeutic management in relation to the phase of the disease. Strategies of professional training should be adopted to notify and expand the provision of information for epidemiological surveillance, aiming to strengthen care, reduce vertical transmission and enable the continuous analysis of this problem.

Objetivo: conhecer a perspectiva dos enfermeiros (as) acerca do sistema de saúde no controle da sífilis. Método: estudo de abordagem qualitativa, por meio de entrevista semiestruturada, foram entrevistadas treze enfermeiras de um hospital secundário em Fortaleza/Ceará, referência para os casos de sífilis, utilizando-se da técnica de análise de conteúdo para análise e interpretação do corpus. Resultados: identificaram-se os núcleos temáticos, na qual destaca-se as categorias: Atenção primária está “furando” e “Há uma falha na educação em Saúde!”. Conclusão: acredita-se na necessidade de novos recursos e ações para os profissionais, principalmente, a partir de intervenções educativas, que focalizem a integralidade da atenção à saúde para a constituição de meios de qualificação do serviço.ABSTRACTObjective: to learn the nurses’ perspective regarding the health system in syphilis control. Se entrevistaron trece enfermeras de un hospital de segundo nivel en Fortaleza/Ceará, de referencia para los casos de sífilis, utilizando la técnica de análisis de contenido para el análisis e interpretación del corpus. Resultados: se identificaron los núcleos temáticos, en los que destacan las categorías: La atención primaria “No cumple lo prometido” y " Hay una falla en la educación de la salud." Conclusión: se cree en la necesidad de nuevos recursos y acciones para los profesionales, principalmente, a partir de las intervenciones educativas que focalicen la totalidad de la atención a la salud para el establecimiento de medios de calificación de servicios.

Factors associated with inadequate follow-up of children with congenital syphilis

OBJECTIVE To analyze factors associated with outpatient follow-up of children with congenital syphilis.METHODS A non-concurrent cohort study performed in primary care units and three reference maternity hospitals in Fortaleza (Ceará State). Data were collected from September 2013 to September 2016 in the notification forms and in the medical records of hospitalization and outpatient follow-up, and they were presented considering an adequate and inadequate follow-up. Children who attended the primary care unit or referral outpatient clinic during the period recommended by the Ministry of Health were considered adequately followed up and performed the recommended examinations. Pearson’s chi-square and Fisher’s exact tests were used in the comparative analysis. The estimated risk of adequate non-follow-up was verified by simple and multiple logistic regression.RESULTS The total of 460 children with congenital syphilis were notified, of which 332 (72.2%) returned for at least one appointment and were part of the study. Exactly 287 (86.4%) children attended the primary health unit; however, there was no reference to congenital syphilis in 236 (71.1%) medical records and no information on the venereal disease research laboratory (VDRL) test was found in 264 (79.5%) children. There was nonadherence to subsequent appointments by 272 (81.9%) individuals. The following variables had a statistically significant association with the non-adequate follow-up of the children: marital status of the mothers, number of prenatal appointments, number of pregnancies, blood count, and radiography of long bones.CONCLUSIONS Most children with congenital syphilis attended primary care for follow-up, but the services do not meet the recommendations of the Brazilian Ministry of Health for adequate follow-up.
