To assess the incidence and intensity of postendodontic pain and flareup in single and multiple visit root canal treatment (RCT) and determine if the difference between the two is significant, a search of PubMed, Medline, Embase, Cochrane, Scopus and Web of science was conducted. The grey literature was searched using Google Scholar and Saudi digital library. Randomised controlled trials evaluating the incidence and intensity of postendodontic pain and flareup published in English from 1 January 2000 to 15 April 2020 were searched. The PRISMA protocol was followed to select the articles. A random effects model was used for the meta-analysis of the data in the included studies. Twenty-one articles were included in the review. Three compared both the incidence and the intensity, while the rest compared either one of the parameters. Most studies used both hand-driven and rotary instruments and irrigated with sodium hypochlorite. Twelve studies used an intracanal medicament. Although individual studies showed discordant treatment outcomes, the meta-analysis did not reveal any significant difference in the incidence or the intensity of the postendodontic flareup between the single and multiple visit RCT groups. Single or a multiple visit root canal treatment is not an independent determinant for the risk of postendodontic pain or a flareup.

Root canal treatment comprises of cleaning and shaping, disinfecting, and root canal obturation. The success of treatment depends on the knowledge of the operator regarding root canal morphology. The mandibular first molars usually have two roots with three canals: distal, mesio-buccal and mesio-lingual. There is a possibility of an additional distolingual root namely radix entomolaris. A 21-year-old male patient was referred to Universitas Gadjah Mada Dental Hospital for root canal treatment. After subjective and objective examination, diagnosis of tooth 36 was pulp necrosis. Dental history of patient revealed caries occurred in left mandibular molar along with spontaneous pain that lasted for hours. The referring dentist had done pulp devitalization and prescribed oral analgesics. Shaping and cleaning were performed on mesio-buccal, mesio-lingual and distal root canal followed by intracanal dressing on first visit. Three days later the patient came with complaint of spontaneous pain. After careful observation, a distolingual root was found, which previously had not been prepared. Root canal preparation was carried out on the distolingual root followed by intracanal dressing. On third visit, the tooth was asymptomatic and root canals were obturated. On fourth visit, the tooth was prepared for onlay restoration and impression was taken. Composite onlay was cemented on the fifth visit. A week later the patient came for recall and no complaints were noted. Conclusion: root canal treatment on mandibular first molars must consider the anatomic variations, such as radix entomolaris. Using proper diagnostic aids and instruments would help identify acessory root canal. A thorough root canal preparation influence the success of root canal treatment.

Aesthetic aspect has become increasingly important in the modern restorative dentistry. Restoration, which considering aesthetics and function, is a paramount importance to the patient, especially in anterior region. Aesthetic analysis consists of facial and space analysis of teeth. Case report. A 23-year-old female patient presented to the Clinic of Conservative Dentistry Dental Hospital of Gadjah Mada University complaining a broken restoration of 12 tooth along with acute pain. Two years earlier, the tooth had been treated endodontically and direct restoration performed. Four days ago, the restoration was broken. A clinical examination revealed that teeth 12 ,11, 21, dan 22 had been previously restored using composite, though it had non-adequate contour and discoloration. Teeth 12 and 11 were tenderness in percussion and enlargement gingiva occurred. A periapical radiograph showed incomplete root canal obturation with apical lesion in teeth 12 and 11. After examination and aesthetic analysis completed, teeth 12 and 11 were carried out root canal retreatment followed by crown lengthening. One week following crown lengthening, teeth 12 and 11 were restored using fiber post and indirect composite crown. Teeth 21 and 22 were restored with direct veneer using composite to achieve ideal width of each tooth. Patient was recalled after two weeks, no complain, asymptomatic teeth and no underfilling were note. Radiographic examination revealed that the diameter of apical lesion became smaller. Conclusion. Aesthetic restoration is important to be conducted following endodontic treatment. Aesthetic restoration is associated with the successful of endodontic treatment in achieving normal function of the teeth.
