“…FNAB is a method used worldwide to diagnose thyroid nodules but it has limitations, such as false-positive ( 1,2 ), FN ( 1-3 ), and inadequate results ( 1,2,4,5 ). The causes of false-positive and FN results are the nature of the lesions, intrinsic limitations of the procedure, and the manner, skill, and experience of the operator ( 7 ).…”

“…The W hen diagnosing focal thyroid nodules, fi ne-needle aspiration biopsy (FNAB) is an essential tool to help decide whether a nodule requires surgery. However, FNAB has inevitable limitations in helping diagnose a thyroid nodule, such as falsepositive ( 1,2 ), false-negative ( 1-3 ), and inadequate ( 1,2,4,5 ) results. Although the Papanicolaou Society of Cytopathology Task Force on Standards of Practice recommends that the false-negative (FN) rate not exceed 2% and the falsepositive rate not exceed 3% for FNAB cytologic evaluation of the thyroid ( 6 ), the reported FN rate of FNAB ranges from 2.3% to 6.2% ( 7-9 ) and the falsepositive rate of FNAB ranges from 0.2% to 11.6% (7)(8)(9)(10)(11).…”

“…2,3 Thus, the commonly used needles would have internal diameters better than 20-50 times that of a follicular cell nucleus. Since three dimensional follicles average around 200 lm, but vary considerably, these commonly used needle sizes often allow passage of intact individual follicles or even small stromal-epithelial fragments (''mini-cores'') on many FNA smears.…”

“…Moreover, a substantial proportion of samples represent "follicular proliferation", neither clearly benign nor malignant and fall into the category of suspicious results. As already noted, about 70% of FNA specimens are classified as benign; in addition, 5% are malignant, 10% are suspicious, and 10% to 20% are non-diagnostic [8][9][10][11][12].…”
