In the late 1980s, thirty years after the Revolution had come to power, Cuban obstetrician Celestino Álvarez Lajonchere recalled the government's early efforts to improve maternal health. In an article for the International Journal of Gynecology & Obstetrics, he wrote that the revolutionary government's goals were "to reduce home births and the dependence on traditional birth attendants (recogedoras)." To accomplish these goals, it established "a Statistical Department . . . to train staff located in the relevant areas of each hospital unit . . . to obtain on-going, comprehensive vital statistics." 1 Lajonchere's unembellished account of the implementation of maternal health programs belies the expansiveness of the projects to which he alluded. Indeed, the government's creation of maternity homes (hogares maternos) for pregnant women and the placement of physicians in rural areas (campo) occurred in the context of broader public health reforms implemented by the Revolution. As for the collection of patient data, that too corresponded with larger efforts to count and categorizethrough censuses and birth recordsboth the bodies and the behavior of Cuban citizens.Targeted processes such as these highlight the Revolution's centralization of medical and state authority as well as its rejection of medical plurality, including the expertise of midwives, Santería priests (santeros), and others. : This article presents the origins and the impact of demographic and socio-economic factors on the process of demographic ageing of the population of Cuba. The main aim of the study was to show the dynamics and regional differences in population ageing based on the W SD dynamic ageing index , as well as the W SE economic ageing index. It was shown that the ageing process reveals moderate diversity of the investigated phenomenon both in the urban-rural dimension as well as at the regional level. The process of population ageing is progressing throughout the whole country and its fastest pace was recorded in the outermost provinces. However, spatial differences in the ageing process may change fundamentally in the years to come, which will result from internal regulations concerning the mobility of the population, and new business investments. As a result, spatial differences are likely to increase and the spatial image of the ageing process will probably change. Głównym celem badań było określenie dynamiki oraz regionalnych różnic w starzeniu się kubań-skiego społeczeństwa w oparciu o wartości wskaźnika starzenia się demograficznego ( W SD ) i wskaźnika starzenia się ekonomicznego ludności ( W SE ). Cuba's accelerated trend to lower fertility and consequent contraction of population reproductive capacity have now become concerns for various social and political actors, including, of course, Cuban demographers, who have been sounding the alarm for more than three decades. The most striking characteristic of Cuba's fertility transition is undoubtedly its abrupt onset, rapidity, and the fact that it took place in the absence of accompanying economic development. Hence the current debate focusing on the roles of the various determinants that led to the transition, and especially of economic factors at different stages, particularly in conditions of heightened population vulnerability during economic crises. This article provides elements to help fi ll gaps in knowledge of the fertility-development relationship today.
