Objective: To know how children with special health needs access the health care network. Method: This is a qualitative research of descriptive-exploratory type, developed using semi-structured interviews mediated by the Talking Map design. Participants were 19 family caregivers of these children in two Brazilian municipalities. Data were submitted to inductive thematic analysis. Results: Difficulties were mentioned from the diagnosis moment to the specialized follow-up, something represented by the itinerary of the c hild and his/her family in the search for the definition of the medical diagnosis and the access to a specialized professional; a gap between the children's needs and the care offered was observed in primary health care. Conclusion: The access of children with special health needs is filled with obstacles such as slowness in the process of defining the child's diagnosis and referral to a specialist. RESUMOObjetivo: Conhecer como se dá o acesso de crianças com necessidades especiais de saúde na rede de atenção à saúde. Método: Pesquisa qualitativa do tipo descritivo-exploratória, desenvolvida por meio de entrevistas semiestruturadas mediadas pelo desenho do Mapa Falante. Participaram 19 familiares cuidadores dessas crianças em dois municípios brasileiros. Os dados foram submetidos à análise temática indutiva. Resultados: Apontou-se dificuldades, do diagnóstico ao acompanhamento especializado, algo denotado pelo itinerário da criança e sua família na busca pela definição do diagnóstico médico e pelo acesso ao profissional especializado; no atendimento na atenção primária à saúde, observou-se um distanciamento entre as necessidades das crianças e a assistência ofertada. Conclusão: O acesso de crianças com necessidades especiais de saúde apresenta-se permeado por obstáculos, como a morosidade no processo de definição diagnóstica da criança e o encaminhamento para o especialista. Houve substituição dos serviços da atenção primária pelos atendimentos em unidades de pronto-atendimento. A Estratégia de Saúde da Família (ESF) é hoje o principal modelo de atenção à saúde do Brasil. Na tentativa de incorporar os princípios dessa estratégia, as atividades do módulo de Práticas de Integração Ensino, Serviço e Comunidade (PIESC), em parceria com uma equipe de Saúde da Família, desenvolveram ações de cuidado integral com famílias vulneráveis. O objetivo deste trabalho é descrever os problemas identificados e as intervenções realizadas por um grupo de estudantes de medicina e seus professores na elaboração do Projeto Terapêutico de uma família acompanhada. Foram identificados, como problemas, a hipertensão arterial sistêmica e obesidade, que se agravaram ao existir sobrecarga do cuidador da família. A adesão terapêutica ainda é um dos principais entraves no êxito das intervenções no âmbito da ESF, mostrando a complexidade do processo saúde-doença-cuidado de cada indivíduo. A clínica ampliada como proposta pedagógica permitiu a identificação de fatores extrínsecos voltados para a compreensão da história individual de cada membro em seus riscos, vulnerabilidades e hábitos de vida assim como o papel do cuidador no processo de aderência, o que não seria possível na clínica tradicional. Políticas de promoção à saúde precisam ser desenvolvidas para a prática do cuidador diante o perfil sociodemográfico e epidemiológico brasileiro.

Social network of children with cronic disease: knowledge and practice of nursing

Objectives:To identify the knowledge and practice of primary care nurses about the social network approach for fa milies of children with chronic diseases. Methods: Qualitative research, conducted by means of interviews with 23 family health nurses, from one municipality in Paraíba and one in Rio de Janeiro, from June to July of 2017. The data were interpreted using thematic analysis. Results: Social network meant institutional support offered by services outside the unit, and socioeconomic problems involved the family context. In practice, nurses find it difficult to provide comprehensive care and establish ties with families. When referring to other professionals, a weakness in the counter-referral to the family health unit is found. Final considerations: Some gaps were found regarding the knowledge and practice of nurses regarding the social network approach, which requires professional training to strengthen social relationships and the necessary support for families of children with chronic diseases.
