Failed Dermal Allograft Procedures for Irreparable Rotator Cuff Tears Can Still Improve Pain and Function: The “Biologic Tuberoplasty Effect”

Abstract: Background:Acellular dermal matrices (ADMs) have been used in the treatment of shoulders with massive rotator cuff tears (MRCTs). Despite clinical improvement, correlation of clinical findings with ADM integrity on imaging has not been investigated.Hypothesis:The pain in shoulders with MRCTs is partially due to bone-to-bone contact between the tuberosity and acromion. Coverage of the tuberosity with an intact graft or a graft that is torn in a way that the tuberosity remains covered will act as an interpositio… Show more

“…The interposition effect of the graft in the setting of an SCR was examined by Mirzayan et al. 31 who coined the term “biological tuberosity effect.” In their article, procedures in which the graft remained attached to the tuberosity had a significantly better outcome than those in which the tuberosity was uncovered. 31 …”

“…The improvement in pain was observed in patients with massive rotator cuff repair after a failed dermal allograft procedure because the tuberosity is covered by the graft to prevent bone-to-bone contact with the acromion. 41 SCR is still evolving from a surgical technique standpoint, with a wide variety of procedures being reported. 2,5,16,19,28,42,53 To date, debate exists about whether a side-to-side graft repair to the adjacent remaining rotator cuff is necessary.…”

“…There were two kinds of tuberoplasty, non-prosthesis tuberoplasty and prosthesis tuberoplasty. Arthroscopic non-prosthesis tuberoplasty was a relatively mini-invasive and effective way among these procedures, which takes little harm to patients [19][20][21][22] . But the problem is that there was not a standard procedure for tuberoplasty, and surgeons had performed this surgery by intuition.…”
