Rev Gaúcha Enferm. 2016 mar;37 (1) Results: There was a predominance of studies by Chinese authors and nurses. The analysis of the studies evidenced that falls followed by fractures lead to dependence of older adults and, consequently, an overload to caregivers. Moreover, older adults and caregivers showed a need for support in the rehabilitation process. Conclusions:Informal caregivers still need to be included in care planning and to be qualified for such care by health professionals, since they positively influence functional independence in the postoperative period. Resultados: Predominaram estudos com autoria de chineses e enfermeiros. A análise dos estudos evidenciou que as quedas seguidas por fraturas promovem a dependência de idosos e, consequentemente, a sobrecarga aos cuidadores, demonstrando o binômio idoso dependente-cuidador necessitar de apoio no processo de reabilitação. Keywords Conclusões:Os cuidadores informais ainda precisam ser inseridos no planejamento e capacitados para o cuidado pelos profissionais da saúde, uma vez que influenciam positivamente a independência funcional no pós-operatório. RESUMEN Objetivo: Verificar la influencia del cuidador informal en la independencia funcional de ancianos en postoperatorio de fractura proximal de fémur por caídas.Método: Revisión integradora, cuyo análisis se reunieron 23 artículos, entre 2002 y 2012, en las bases de datos Literatura Latinoamericana y del Caribe en Ciencias de la Salud, Cumulative Index to Nursing and Allied Health Literature, US National Library of Medicine e Scopus.Resultados: Predominaron estudios con autores chinos y enfermeros. El análisis de los estudios evidenció que las caídas que producen fracturas promueven la dependencia de ancianos y, consecuentemente, sobrecarga en los cuidadores, demostrando la dupla anciano dependiente-cuidador necesidad de apoyo en el proceso de rehabilitación. Conclusiones:Los cuidadores informales necesitan inserción en la planificación, además de capacitación para cuidados por parte de profesionales de salud, toda vez que influyen positivamente en la independencia funcional durante el postoperatorio.Palabras clave: Anciano. Objectives: Hip fractures are one of the most serious causes of impairment and death in the elderly. The aim of this study is to identify the effects of different risk factors on mortality after a hip fracture in elderly Turkish patients.Patients and Methods: Two-hundred forty patients (>60 years; mean age 76.0±8.2 years) who were operated for fracture of the hip from 2001 to 2006 were investigated. Demographic data, preoperative length of hospital stay, anesthesia type and ASA physical status, albumin level at admission and comorbidities were recorded.Results: Mean albumin level was 3.45±0.62 g/dl. Onehundred eight (45%) had hypoalbuminemia. The difference between mortality rates of normo-and hypoalbuminemic patients were not significant at one and two-year. According to preoperative length of stay; one-year mortality was significantly higher in male patients (p=0.015) while two-year mortality was not. One-year mortality was related with high ASA physical status scores (p=0.002) while two-year mortality was not (p=0.169). According to gender; one-year mortality was found higher in men (p=0.001).Conclusion: Due to comorbidities, high ASA scores are common in elderly patients. Since the preoperative evaluation in high ASA physical status is longer, this may prolong the preoperative hospital stay. Delay after admission and high ASA physical status are important predictors of mortality for elderly hip fracture patients.Key words: Hip fracture; mortality; ASA; albumin.Amaç: Kalça kırıkları ileri yaşların fonksiyon kaybı ve ölüm ile sonuçlanabilen ciddi sağlık sorunlarındandır. Bu çalışmada amaç kalça kırığı sonrası yaşlı Türk hastalarda değişik risk faktörlerinin mortalite üzerine etkisini ortaya koymaktır. Bulgular: Geliş albumin seviyesi 3.45±0.62 g/dl olarak bulundu. Yüz sekiz hastada (%45) albumin düşüklüğü vardı. Albumin düzeyine göre bir ve iki yıl mortalite oranı anlamlı değildi. Ameliyat öncesi yatış süreleri incelendiğinde; birinci yıl mortalitesi erkek hastalarda istatistiksel anlamlı yüksek bulunurken (p=0.015), ikinci yıl mortalitesi bulunmadı. ASA fiziksel skorları yüksek olan hastaların birinci yıl mortaliteleri ile korelasyonunun istatistiksel olarak anlamlı olduğu (p=0.002), ikinci yılda ise olmadığı (p=0.169) saptandı. Cinsiyet incelendiğinde, birinci yıl mortalitesi erkekler için yüksek bulundu (p=0.001).Sonuç: Yüksek ASA skorlarına ek morbiditeler nedeniyle yaşlı hastalarda sık karşılaşılmaktadır. Preoperatif hazır-lık sürecinin ASA skoru yüksek olan hastalarda uzun olması nedeniyle ameliyat öncesi süre uzayabilmektedir. Kalça kırığı olan hastaların bir ve iki yıllık mortalite öngörüsünde ASA fiziksel durumu ve ameliyat öncesi gecikmenin önemli belirteçler olduğu görüşündeyiz.

Moderate to severe depressive symptoms and rehabilitation outcome in older adults with hip fracture

Objective: To evaluate if depressive symptoms affect recovery of walking ability and 1-year institutionalization or mortality in older adults who underwent post-hip fracture (HF) surgery rehabilitation. Methods: Depressive symptoms were assessed on admission using the 15-item Geriatric Depression Scale (GDS), with scores !10/15 indicating moderate to severe depressive symptoms. Multidimensional assessment included Mini Mental State Examination, Charlson Comorbidity Index, Body Mass Index, albumin serum levels, number of drugs, antidepressants and Barthel Index (BI) on admission and at discharge. Walking ability was evaluated using the BI walking sub-item referred to 1 month before HF, on admission, and at discharge. Patients scoring 3/15 BI walking sub-item on admission (i.e. those fully dependent or requiring major supervision in walking) were included. Walking independence at discharge was defined as a score !12/15 at the BI walking sub-item. Results: In multivariate analyses, after adjustment for covariates and potential confounders, patients with moderate to severe depressive symptoms were more likely to fail walking independence at discharge (odds ratio, OR ¼ 3.2; 95% CI ¼ 1.3 to 7.8; p ¼ 0.010) and to be institutionalized or died at 1 year (OR ¼ 3.6, 95% CI ¼ 1.4 to 9.1, p ¼ 0.007). In further analyses, the failure to recover walking independence at discharge partly mediates the relationship between moderate to severe depressive symptoms and 1-year adverse events. Conclusions: Moderate to severe depressive symptoms affect the recovery of walking independence after HF rehabilitation and are associated with severe adverse outcomes at 1 year.
