Uterine myomas represent benign tumors of the myometrium, and they develop frequently in women of reproductive age. The frequency of uterine myomas increases with age until women reach the menopause. The study included patients with a myomatous uterus, in the reproductive age or peri-menopausal age, independently evaluating small and large myomas. Myometrial alterations in their direct vicinity were evaluated independently of the myomas. The study included evaluation of immunolocalization of two index proteins which participate in myoma cells growth control: Ki-67 nuclear antigen and caspase 3. In women of reproductive age, both in small and large myomas, elevated immunostaining of Ki-67 was noted in parallel to low levels of caspase 3 staining, which indicated the ongoing process of proliferation. In women of peri-menopausal age with small or large myomas, no Ki-67 immunostaining was detected, while staining of caspase 3 manifested low levels. ABSTRACT. We conducted a meta-analysis to examine p16INK4a expression in uterine smooth muscle tumors (USMTs). Although the prognostic value of tumor suppressor p16INK4a has been elucidated in a variety of cancers and precancerous lesions, its role in USMTs is not well established. We searched PubMed, Web of Science, and Embase for publication son p16INK4a expression in USMTs. Strict inclusion and exclusion criteria were imposed. Risk ratios (RRs) with 95% confidence intervals (95%CIs) were calculated to assess the strength of association. Publication bias was estimated using funnel plots and the Egger's regression test. Twelve eligible studies comprising 661 patients were included. Compared with leiomyoma (LM), the figures for the strength of association were as follows: LM variants (RR = 1.53, 95%CI = 1.03-2.27, P = 0.036, random effect); leiomyosarcoma (LMS) (RR = 3.20, 95%CI = 1.68-6.12, P < 0.001, random effect); and smooth muscle tumors of uncertain malignant potential (STUMP) (RR = 2.90, 95%CI = 1.17-7.21, P = 0.022, random effect). p16INK4a expression was significantly higher in LMS than in LM variants (RR = 3.74, 95%CI = 1.96-7.13, P < 0.001, random effect) or STUMP (RR = 1.67, 95%CI = 1.26-2.23, P < 0.001, fixed effect). There was a significant correlation between overexpressed p16INK4a and recurrence rates of USMTs (RR = 1.85, 95%CI = 1.11-3.10, P = 0.019, fixed effect). p16INK4a over expression is a potential biomarker for diagnosing problematic USMTs and it might indicate a worse prognosis. However, there is currently insufficient evidence to assess the prognostic value of p16INK4a in USMTs.

менным методам исследования, все чаще диагностирует-ся у женщин репродуктивного возраста и нередко сопро- Цель исследования -изучение клинико-морфологических особенностей лейомиомы тела матки у женщин раннего репродуктивного возраста, включая исследование экспрессии рецепторов эстрогена, прогестерона, определение про-лиферативного индекса, а также подсчет сосудов капиллярного типа в опухолевых узлах. Материал и методы. Иммуно-гистохимическое исследование 20 удаленных узлов лейомиом (у всех женщин миомэктомия была произведена в фолли-кулярную фазу цикла) было проведено у 8 пациенток в возрасте 24-30 лет, нормального телосложения, с анамнестиче-скими указаниями на отсутствие родов и абортов. Исследовали 3-4 образца опухолевой ткани из каждого узла и фраг-менты прилежащего миометрия, а также экспрессию рецепторов эстрогена, прогестерона, Ki-67, CD31. Количественную оценку степени экспрессии рецепторов эстрогена и прогестерона проводили полуколичественным методом по системе Hystochemical score. Интенсивность экспрессии рецепторов CD31 оценивали отдельно по 3-балльной шкале, определе-ние индекса пролиферации-экспрессии рецепторов Ki-67 проводили путем подсчета позитивно окрашенных ядер любой интенсивности на 100 ядер. Результаты. В клетках лейомиомы по сравнению с прилежащим неизмененным миометрием наблюдалась более высокая экспрессия рецепторов эстрогена и прогестерона, при этом экспрессия рецепторов про-гестерона превышала таковую рецепторов эстрогена, а пролиферативная активность (Ki-67) была обнаружена только в лейомиоме и отсутствовала в здоровом миометрии. Васкуляризация была вариабельной как в опухоли, так и в неизме-ненном миометрии, наиболее выраженная экспрессия рецепторов прогестерона сопровождалась большим количеством капилляров в опухолевой ткани, что сочеталось с повышенным индексом пролиферации клеток. Заключение. Накопление фактического материала в отношении молекулярных основ развития лейомиомы будет способствовать разработке раци-ональной терапии и хирургии данной опухоли.Авторы информируют об отсутствии конфликта интересов.
