Autosomal dominant polycystic kidney disease (ADPKD), the most common form of polycystic kidney disease (PKD), is a disorder with characteristics of neoplasia. However, it is not known whether renal transplant recipients with PKD have an increased risk of cancer. Data from the Scientific Registry of Transplant Recipients, which contains information on all solid organ transplant recipients in the United States, were linked to 15 population-based cancer registries in the United States. For PKD recipients, we compared overall cancer risk with that in the general population. We also compared cancer incidence in PKD versus non-PKD renal transplant recipients using Poisson regression, and we determined incidence rate ratios (IRRs) adjusted for age, sex, race/ethnicity, dialysis duration, and time since transplantation. The study included 10,166 kidney recipients with PKD and 107,339 without PKD. Cancer incidence in PKD recipients was 1233.6 per 100,000 person-years, 48% higher than expected in the general population (standardized incidence ratio, 1.48; 95% confidence interval [95% CI], 1.37 to 1.60), whereas cancer incidence in non-PKD recipients was 1119.1 per 100,000 person-years. The unadjusted incidence was higher in PKD than in non-PKD recipients (IRR, 1.10; 95% CI, 1.01 to 1.20). However, PKD recipients were older (median age at transplantation, 51 years versus 45 years for non-PKD recipients), and after multivariable adjustment, cancer incidence was lower in PKD recipients than in others (IRR, 0.84; 95% CI, 0.77 to 0.91). The reason for the lower cancer risk in PKD recipients is not known but may relate to biologic characteristics of ADPKD or to cancer risk behaviors associated with ADPKD.

Palyginti pacientų, sergančių autosomine dominantine policistine inkstų liga, kuriems prieš inkstų transplantaciją buvo atlikta nefrektomija, su pacientais, kuriems nefrektomija nebuvo atlikta, potransplantacines išeitis, natyvinių inkstų dydžių pokyčius. Nustatyti, ar nefrektomija, atlikta prieš inksto transplantaciją, gali pagerinti tolesnę autosominės dominantinės policistinės inkstų ligos eigą. Tyrimo medžiaga ir metodika. Atliktas retrospektyvinis tyrimas, analizuoti 739 pacientai, kuriems atlikta inksto transplantacija 14,5 metų laikotarpiu. Atrinkti 53, sergantys autosomine dominantine policistine inkstų liga. Į galutinę analizę pateko 50 pacientų. Visi tiriamieji pagal tai, ar jiems buvo atlikta nefrektomija prieš inksto transplantaciją, suskirstyti į dvi grupes: A grupė -nefrektomija prieš transplantaciją buvo atlikta ir B grupė -nefrektomija prieš transplantaciją nebuvo atlikta. Statistinė analizė atlikta SPSS 20.0 programa (statistiškai reikšminga, kai p < 0,05). Rezultatai. A grupę sudarė 26, B grupę -24 pacientai. Abi grupės statistiškai reikšmingai nesiskyrė pagal recipientų demografinius duomenis, dializių trukmę iki transplantacijos, inksto donoro tipą, lėtinio transplantuoto inksto nepakankamumo dažnį bei inkstų funkcijos atsikūrimą po transplantacijos per ≥5 metus. Kepenų cistų dažniau pasitaikė A grupėje (88,5 proc. vs 62,5 proc., p = 0,032). Iš pooperacinių komplikacijų tik hipertenzija dažnesnė B grupėje (95,8 proc. vs 69,2 proc., p = 0,014). Nepašalintų policistinių inkstų echoskopinių matavimų rezultatai: visos imties pacientų inkstų ilgio ir pločio matmenys po transplantacijos sumažėjo statistiškai reikšmingai (ilgio -20,0 ± 35,8 mm, p = 0,026; pločio -16,8 ± 22,9 mm, p = 0,001). Grupėse atskirai stebėtas statistiškai patikimas inkstų pločio matmenų po transplantacijos sumažėjimas (atitinkamai 40,2 ± 27,3 mm, p = 0,03 ir 11,0 ± 18,1 mm, p = 0,014), o grupėje, kurioje vienpusė nefrektomija prieš transplantaciją buvo atlikta, šis sumažėjimas buvo žymesnis (p = 0,041). Transplantuoto inksto ir recipientų išgyvenamumas abiejose grupėse statistiškai reikšmingai nesiskyrė (transplantuoto inksto išgyvenamumas atitinkamai A ir B grupėse 4,0 ± 3,6 vs 4,4 ± 3,0 metų, p = 0,544; recipientų -4,6 ± 3,7 vs 5,6 ± 2,7 metų, p = 0,592). Išvados. Bendra pooperacinė eiga tarp grupių reikšmingai nesiskyrė, tačiau hipertenzija B grupėje pasireiškė dažniau. Tyrimas parodė, kad po transplantacijos natyvinių inkstų matmenys reikšmingai sumažėja. Be to, atlikta nefrektomija nėra susijusi su geresniu transplantuoto inksto bei recipiento išgyvenamumu.
