Exosomal ncRNAs: The pivotal players in diabetic wound healing

Abstract: Diabetes is the most prevalent metabolic disease in the world today. In addition to elevated blood glucose, it also causes serious complications, which has a significant effect on the quality of life of patients. Diabetic trauma is one of complications as a result of the interaction of diabetic neuropathy, peripheral vascular disease, infection, trauma, and other factors. Diabetic trauma usually leads to poor healing of the trauma and even to severe foot ulcers, wound gangrene, and even amputation, causing ser… Show more

“…As natural carriers of cargo, exosomes exhibit great potential for therapeutic delivery. Exosomes show high biocompatibility and stability and low immunogenicity and toxicity [112][113][114]. Exosomes have inherent biochemical characteristics that contribute to their extensive application in drug-delivery systems (figure 3).…”

“…A angiogênese é uma importante causa de cicatrização de feridas diabéticas, pelo ressurgimento de novos vasos sanguíneos, mas, nos casos de úlceras diabéticas o grande potencial é o dano às células endoteliais vasculares. O microRNA, especialmente o miR-212-3p no diabetes causa disfunção das células β pancreáticas e inibe a autofagia, ou seja, o potencial de regeneração celular (Shen et al, 2022).…”

“…Normal wound healing consists of four successive and overlapping stages: hemostasis, inflammation, proliferation, and remodeling, which involve many types of cells and cytokines ( 9 ). However, diabetic wound healing usually does not progress due to the complex microenvironment with hyperglycemia, hypoxia, poor angiogenesis, and bacterial infection ( 10 ). Conventional treatment, including debridement, dressing, wound off-loading, infection control, vascular assessment, and glycemic control were ineffective, with 14%–20% of patients eventually undergoing low limb amputations ( 11 , 12 ).…”
