Effects of Dietary Phytoestrogens on Hormones throughout a Human Lifespan: A Review

Dietary phytoestrogens are bioactive compounds with estrogenic activity. With the growing popularity of plant-based diets, the intake of phytoestrogen-rich legumes (especially soy) and legume-derived foods has increased. Evidence from preclinical studies suggests these compounds may have an effect on hormones and health, although the results of human trials are unclear. The effects of dietary phytoestrogens depend on the exposure (phytoestrogen type, matrix, concentration, and bioavailability), ethnicity, hormone levels (related to age, sex, and physiological condition), and health status of the consumer. In this review, we have summarized the results of human studies on dietary phytoestrogens with the aim of assessing the possible hormone-dependent outcomes and health effects of their consumption throughout a lifespan, focusing on pregnancy, childhood, adulthood, and the premenopausal and postmenopausal stages. In pregnant women, an improvement of insulin metabolism has been reported in only one study. Sex hormone alterations have been found in the late stages of childhood, and goitrogenic effects in children with hypothyroidism. In premenopausal and postmenopausal women, the reported impacts on hormones are inconsistent, although beneficial goitrogenic effects and improved glycemic control and cardiovascular risk markers have been described in postmenopausal individuals. In adult men, different authors report goitrogenic effects and a reduction of insulin in non-alcoholic fatty liver patients. Further carefully designed studies are warranted to better elucidate the impact of phytoestrogen consumption on the endocrine system at different life stages.

Adiponectin (ApN) is a hormone abundantly secreted by adipocytes and it is known to be tightly linked to the metabolic syndrome. It promotes insulin-sensitizing, fat-burning, and anti-atherosclerotic actions, thereby effectively counteracting several metabolic disorders, including type 2 diabetes, obesity, and cardiovascular diseases. ApN is also known today to possess powerful anti-inflammatory/oxidative and pro-myogenic effects on skeletal muscles exposed to acute or chronic inflammation and injury, mainly through AdipoR1 (ApN specific muscle receptor) and AMP-activated protein kinase (AMPK) pathway, but also via T-cadherin. In this review, we will report all the beneficial and protective properties that ApN can exert, specifically on the skeletal muscle as a target tissue. We will highlight its effects and mechanisms of action, first in healthy skeletal muscle including exercised muscle, and second in diseased muscle from a variety of pathological conditions. In the end, we will go over some of AdipoRs agonists that can be easily produced and administered, and which can greatly mimic ApN. These interesting and newly identified molecules could pave the way towards future therapeutic approaches to potentially prevent or combat not only skeletal muscle disorders but also a plethora of other diseases with sterile inflammation or metabolic dysfunction.

Настоящая работа посвящена вопросам фармакологических особенностей и преимуществ использования антагониста рецепторов ангиотензина II телмисартана в реальной клинической практике. Данный бифункциональный представитель класса блокаторов рецепторов ангиотензина II первого типа обладает рядом уникальных фармакокинетических особенностей: наибольшей продолжительностью действия, высокой степенью липофильности и максимальной эффективностью в отношении активации рецепторов PPARγ. Представлены результаты многочисленных исследований, подтверждающих высокую эффективность телмисартана и способность контролировать артериальное давление в течение суток и ранние утренние часы по сравнению с другими антигипертензивными препаратами. Основные плеойтропные свойства телмисартана доказаны на большинстве этапов кардиоваскулярного и ренального континуумов, главными из которых являются снижение инсулинорезистентности, повышение уровня адипонектина, улучшение функциональной активности эндотелия, ангио и ренопротективные эффекты и способность обеспечивать регресс гипертрофии миокарда левого желудочка. Доказана способность телмисартана влиять на прогноз у пациентов высокого сердечно-сосудистого риска и обеспечивать снижение летальности и количества сердечно-сосудистых осложнений. Опираясь на результаты масштабных исследований, рассмотрены возможности использования телмисартана 80 мг/сут., в т. ч. в сочетании с гидрохлоротиазидом, как наиболее мощной двойной фиксированной комбинации и одной из основных современных стратегий антигипертензивной терапии. Ключевые слова: артериальная гипертензия, высокий сердечнососудистый риск, комбинированная антигипертензивная терапия, фиксированные комбинации, поражение органов-мишеней, блокаторы РААС, телмисартан, тиазидные диуретики.
