785Brusellozlu 21 Olgunun Değerlendirilmesi Ö ÖZ ZE ET T A Am ma aç ç: : Bu çalışmanın amacı, brusellozlu hastaların epidemiyolojik, klinik ve laboratuvar bulgularını, komplikasyonlarını ve tedavilerini değerlendirmektir. G Ge er re eç ç v ve e Y Yö ön nt te em ml le er r: : Bu çalışmada Gaziantep Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Kliniği'nde Temmuz 2010-Temmuz 2012 tarihleri arasında bruselloz tanısı ile izlenen 21 hasta retrospektif olarak değerlendiril-miştir. Bruselloz tanısı klinik bulgularla birlikte serum brusella aglütinasyon titresinin 1:160 veya üzerinde olması ve/veya kültürde organizmanın izole edilmesi ile konulmuştur. B Bu ul lg gu ul la ar r: : Olguların 9'u erkek 12'si kızdır; ortanca yaşları 8,96±4,07 yıldır (2-15 yıl). Hastaların başvuru sırasındaki en sık yakınmaları eklem ağrısı ve/veya şişliği (%76), ateş (%52), halsizlik (%52), iştahsızlık (%33), terleme (%23), en sık saptanan fizik muayene bulguları artrit (%61), ateş (%52) ve hepatomegali (%14) olarak belirlenmiştir. Olguların 18'i akut, 2'si subakut, 1'i kronik olarak değerlendirilmiştir. Laboratuvar incelemeleri yapılan hastaların %21'inde aspartat aminotransferaz, %26,31'inde alanin aminotransferaz düzeylerinde artış, %38'inde eritrosit sedimentasyon hızında ve %76'sında C-reaktif protein düzeylerinde artış saptanmıştır. İki olguya nörobruselloz tanısı konulmuştur. Olgulardan birisinin beyin manyetik rezonans incelemesinde subkortikal beyaz cevherde hiperintens alanlar saptanmıştır. Vakaların tümüne kombine antibiyotik tedavisi uygulanmıştır. Bir hastada (%4,76) rölaps görülmüştür. Brucellosis was diagnosed by an increased brucella agglutination titer of 1:160 or greater and/or isolation of the organism by culture in the presence of clinical findings. The most common symptoms on admission were joint pain and swelling (76%), fever (52%), malaise (52%), lack of appetite (33%), sweating (23%), and on physical examination, the most common findings were arthritis (61%), fever (52%), and hepatomegaly (14%). If immediately following a traumatic event, the detached fragments is gathered and brought to the emergency room with the patient, the fragment reattachment should be considered one option for the treatment plan. Store the tooth fragment in an correct solution, apply an appropriate resin composite material and add a proper mechanical retention, may offer higher esthetic result, better bond strength and improved clinical longevity, compared with other types of restorative procedures.

Int. J. Odontostomat., 6(1):27-37, 2012. SUMMARY:Emergency treatment of 11-years-old female patient, presenting a complicated crown root fracture, which simultaneously presented oblique root fracture in the maxillary right central incisor. In order to expose the subgingival extension of the fracture, it was necessary to raise a mucoperiosteal flap. In light of pulp exposure, and prior to the repositioning of fragments with adhesive composite resin technique, Cvek pulp therapy was performed . Despite the existence of a 4-5 mm subgingival extension, neither surgical nor orthodontic extrusion of the root fragment was performed due to the presence of intra-alveolar oblique root fracture without displacement. Minimally invasive and conservative clinical management is basic, namely due to the great capacity of pulp healing in young permanent teeth, the absence of displacement between fragments of root fracture, and great capacity of adhesion and tensile strength of current adhesive systems. Clinical and radiographic controls over the first 18 months have shown an excellent pulp response, with some minor periodontal complications in relation to the biological width invasion and an adequate functional and aesthetic result.
