Stress, anxiety and depressive disorders are often characterized by the activation of the stress axis, which results in similar symptoms at some point in these disorders. These disorders are closely related to each other—they occur simultaneously or follow one another. The diagnosis of stress, anxiety and depression is not a perfect procedure currently—it is based on patient observation and an interview with the patient and their family. There are no laboratory tests that would dispel the doubts of the doctor making the diagnosis and allow the appropriate treatment to be implemented as soon as possible. Therefore, this study will review the components of saliva that could be helpful in the quick diagnosis of stress, anxiety and/or depression. Such potential salivary biomarkers could also be useful in monitoring the effectiveness of pharmacological treatment prescribed by a psychiatrist. The following are promising salivary biomarkers of stress, anxiety or depression: cortisol, immunoglobulin A (sIgA), lysozyme, melatonin, α-amylase (sAA), chromogranin A (CgA) and fibroblast growth factor 2 (FGF-2). To the best valuable potential salivary markers of stress, we can include cortisol, lysozyme, sAA and CgA. To differentiate depression from stress, salivary cortisol and melatonin can be helpful. Fluctuations in the concentrations of the above-mentioned substances in saliva indicate a particularly strong relationship with typical human psychological problems, such as stress, depression or anxiety.

Background: Physiological stress due to physical activity associated with mental stress can affect the responses of the sympathoadrenal axis and the hypothalamic-pituitary axis. Objectives: The present study aimed to investigate the effect of exercise with mental stress on cortisol and alpha-amylase changes in young men. Methods: In a quasi-experimental study, 20 inactive healthy men were randomly selected and divided into two groups of exercise and exercise with mental stress. The exercise group pedaled on a fixed bike at an intensity of 60% of maximal aerobic power for 37 minutes and the exercise group with mental stress received 20 minutes of mental stress (from 12th minutes to 32nd minutes) during the 37 minutes of pedaling. Salivary samples were collected before and immediately after the end of the exercise. Results: The exercise had a significant effect on increasing salivary cortisol (P = 0.007) and alpha-amylase (P = 0.01). Exercise with mental stress had a significant effect on salivary cortisol (P = 0.004) and alpha-amylase (P = 0.04). The exercise and exercise with mental stress had the same effects on increasing salivary cortisol (P = 0.42) and alpha-amylase (P = 0.83). Conclusions: It seems that exercise and exercise with mental stress have the same effects on increasing salivary cortisol and alphaamylase levels.

Objetivo: Verificar se existe associação entre a α-amilase salivar, a escala hospitalar de ansiedade e a depressão e autorrelato de estudantes universitários com sintomas da ansiedade. Métodos: Estudo observacional, realizado com 67 estudantes de uma universidade pública. Para a coleta de dados, foram utilizada a Escala Hospitalar de Ansiedade e Depressão, o autorrelato de ansiedade e a aferição da α-amilase salivar por meio do dispositivo Cocoro Meter®. Os dados foram analisados por meio do teste de qui-quadrado. Resultados: Não houve associação entre α-amilase salivar e a Escala Hospitalar de Ansiedade e Depressão (p=0,51), nem entre a α-amilase salivar e o autor- relato de ansiedade (p=0,51), o que indica que o biomarcador não produz resposta dessas duas variáveis ao mensurar a ansiedade. Conclusão: O biomarcador α-amilase salivar não possui a mesma capacidade em mensurar a ansiedade quando associado com à Escala Hospitalar de Ansiedade de Depressão e ao autorrelato em estudantes universitários com sintomas de ansiedade.
