Evaluation of Pipeline Flex delivery system for the treatment of unruptured aneurysms

Abstract: Refinements in endovascular technology have revolutionized the treatment of intracranial aneurysms (IAs) with the development of flow-diversion technology. The first generation of the Pipelin Embolization Device (PED) has demonstrated its safety and efficacy. However, the deployment technique was a difficult task that often led to complex maneuvers. The Pipeline Flex Embolization Device (PED Flex) is the second generation and its introduction has arrived with high expectations due to a completely redesigned de… Show more

“…Mortality rates ranged from 0% to 2.3%,1 2 8 27 28 and occlusion rates ranged from 70.0% to 91.7% 1 2 5 6 8 27 28. One major international retrospective study, IntrePED, analyzed the results of 793 patients with 906 aneurysms treated with PEDs in 17 centers 15. There were 473 (52.2%) small aneurysms (<10 mm), 349 (38.5%) of which were located in the ICA.…”

“…Even with a high rate of complex cavernous geometry, balloon angioplasty was used to improve PED vessel wall apposition in only 8 cases (7%). This low rate of balloon angioplasty for postprocessing the PED is a function of the deployment characteristics of PED Flex [8,10,11], microcatheter selection, and the stability plus trackability of Cat5.…”

“…35 Outcome data for the Pipeline Flex are still limited, but initial results support its safety and short-term efficacy. 36 High rates of occlusion with low rates of short-and long-term disability have been reported from several selected small studies, as follows: in 10 patients with 11 anterior saccular aneurysms, 12 in 42 patients with 44 predominantly anterior saccular aneurysms, 37 and in 30 patients with 30 predominantly anterior saccular aneurysms. 38 The Pipeline Flex device has been shown to reduce total procedure and fluoroscopy times and the rate of deployment failure compared with the original PED.…”
