The objectives of the study were to examine the initial, first-year anti-rheumatic outpatient therapy in patients with incident SLE, as well as the concomitant use of drugs for certain comorbidities, compared to the use in the general population. The Finnish nationwide register data on special reimbursements for medication costs was screened to identify the inception cohort of 566 adult SLE patients (87% females, mean age 46.5 AE 15.9 years) over the years [2000][2001][2002][2003][2004][2005][2006][2007]. The patients were linked to the national Drug Purchase Register. Of those, 90% had purchased at least once some disease-modifying anti-rheumatic drugs (DMARDs) during the first year. Hydroxychloroquine was the most common (76%), followed by azathioprine (15%) and methotrexate (13%). With the exception of increase in mycophenolate mofetil, the proportions remained stable over the whole study period 2000-2007. Drugs for cardiovascular diseases, dyslipidemia, diabetes mellitus, hypothyroidism and obstructive pulmonary disease were more frequently purchased than in the sex-and age-adjusted population, with rate ratios ranging from 1.6 to 7.8. Over the years 2000-2007, almost all the patients with incident SLE in Finland started with a DMARD. Higher percentages of SLE patients were on medication for several common chronic diseases than in the population as a whole. Lupus (2016) 25, 666-670.

IntroductionTreatment adherence plays a large role in chronic dermatologic diseases and may play an important role in the outcomes of patients with cutaneous lupus erythematosus (CLE). We sought to gauge what is currently known about adherence to topical and oral medications in patients with CLE.MethodsA review of MEDLINE was performed using a combination of the phrases “adherence”, “compliance”, “lupus”, and “cutaneous”. Studies were hand searched and prospective and cross-sectional studies evaluating medication adherence in patients with CLE and systemic lupus erythematosus (SLE) were included.ResultsOnly two articles explored adherence in patients with CLE, while 17 articles discussed treatment adherence in patients with SLE. Depression was consistently cited as detrimental to adherence. The impact that race, ethnicity, and education has on adherence is unclear. Three studies noted a clear connection between adherence and disease activity, while two others did not. Few studies investigated methods that have improved adherence to treatment which have showed promise.ConclusionMuch of what we know about adherence to medication in patients with lupus is limited to SLE. Although cutaneous symptoms are among the most common manifestations of SLE, cutaneous disease is often managed at least in part with topical agents, and adherence to topical treatment was not assessed in any of the articles, though one study investigated sunscreen usage in patients with CLE. ______________________________________________________________________RESUMO: Lúpus eritematoso sistêmico (LES) é uma doença autoimune e multissistêmica. Ocorre predominantemente no gênero feminino. O tratamento é diversificado e contém regras complexas, dificultando a adesão. Esta pesquisa estudou os efeitos de três condições de intervenção sobre comportamentos de adesão ao tratamento em mulheres com LES. Trata-se de um estudo longitudinal com coleta de dados em intervalos de 30, 60, 90 e 120 dias. Participaram quinze pacientes com diagnóstico de LES, em seguimento no ambulatório de um hospital da rede pública de saúde. Utilizou-se: formulário de observação da consulta médica; protocolo de análise de prontuário; roteiros de entrevistas; checklist sobre adesão ao tratamento; formulários de registro de automonitorização; escala analógica visual de dor; Inventário Beck para depressão (BDI); questionário de avaliação de qualidade de vida SF-36 e o índice de atividade da doença (SLEDAI). As participantes foram distribuídas sequencialmente em três condições de intervenção: Condição A (n=5), rotina e entrevista; Condição B (n=5), rotina e entrevista com uso de checklist; Condição C (n=5), rotina, entrevista com uso de checklist e registros de automonitorização. Fez-se análise quanti-qualitativa, comparando-se os dados nos diferentes momentos do estudo. Como resultado observouse que, independentemente da condição à qual a participante foi exposta, houve melhora na adesão relatada, bem como no estado clínico e na qualidade de vida das participantes ao final do estudo; entretanto, as participantes da Condição C mantiveram os comportamentos de adesão instalados mesmo após a retirada do procedimento de intervenção, sugerindo melhor índice de adesão ao tratamento (média de 95,71).
