Rationale, aims, and objectives Expanded financial coverage is critical to fight against poverty through public health reforms in developing countries. This study explores inequity in public health financing reforms in Turkey between 2003 and 2015. Methods This paper has two parts. The first part examines inequity in health care financing in Turkey between 2003 and 2015. Gini, entropy (Theil and mean logarithmic deviation), and Atkinson indexes were calculated. In the second part of the paper, we investigated the degree of progressivity by using Kakwani index and Lorenz and concentration curves. Results We found a decreasing trend in terms of inequity. After major public health reforms and unification of the health financing system, it is seen that the distribution of out‐of‐pocket expenditure on health stands on the shoulders of vulnerable groups. Conclusions Study results provide a deep understanding of the effects of poverty on public health financing reforms on households in Turkey. To reduce out‐of‐pocket health spending inequities and to protect vulnerable groups from increasing the level of health expenditures, we suggest that the government enlarges health insurance coverage for the poor.

ÖzetBu çalışmanın amacı Türkiye'de sektörel verimliliğin bölgeler arası analizinin gerçekleştirilmesidir. Analizde kullanılan sektörel istihdam ve üretim verileri düzey 2'ye göre 2014 yılı için 26 bölgede TUİK raporlarından derlenmiştir. Üç aşamalı istatistik yöntemler kullanılmıştır: İlk aşamada Atkinson bölgesel eşitsizlik endeksi yöntemi, ikinci aşamada lokasyon katsayısı yöntemi ve son aşamada da PearsonSpearman-Kendall korelasyon katsayıları analizleridir. Atkinson bölgesel eşitsizlik endekslerine göre, 26 bölge arasında sektörel istihdamın sektörel üretime göre en dengesiz dağılarak en fazla sosyal refah kaybına yol açtığı sektör tarım sektörü olarak tespit edilmiştir. Sosyal refah kaybı tarımda %19,7 iken sanayi sektöründe %5 ve hizmetler sektöründe 6,6'dır. Bunun diğer anlamıysa sektörel verimlilikte bölgelerarası eşitsizlik en fazla tarımdadır. İkinci aşamada istihdamın ve üretimin sektörlere göre bölgesel uzmanlaşması analiz edilmiştir. Analizde tarım çalışanlarında 15 bölgede, sanayi çalışanlarında 4 bölgede görülmüşken hizmetler sektöründe çalışanlarda hiçbir bölgede bölgede uzmanlaşma görülmemiştir. Sektörel üretime göreyse tarım üretiminde 18 bölgede, sanayi üretiminde 4 bölgede görülmüşken hizmetler sektöründe üretimde hiçbir bölgede uzmanlaşma görülmemiştir. Üçüncü ve son aşamada gerçekleştirilen korelayon analizlerindeyse elde edilen bulgulara göre: Sektörel istihdam ve üretime göre bölgesel uzmanlık katsayıları (LQ) arasında korelasyon oranları en yüksek %80 ile sanayi sektöründe gerçekleşmiştir. Bu oranlar hizmetler sektöründe %41,4; tarım sektöründeyse %-44,6'dır. Her üç analiz bulguları, bölgelerarası en dengesiz verimliliğin tarım sektöründe gerçekleştiğini göstermektedir. Verimlilik farklılıklarında, kısmen geçimlik ekonominin varlığı, bölgelere göre tarım ürün çeşitliliği, makineleşme farklılığı ve arazi verimindeki farklılığın etkileri olarak görülebilir. Sanayi sektöründe ise istihdam edilenlerin verimlilikleri, diğer sektörlerle karşılaştırılamayacak kadar yüksek düzeydedir. BackgroundThe energy transition pattern in India highlights that cooking continues to be the weakest link in the energy transition process for rural households. The government is trying to subsidize LPG for rural families to increase usage of LPG in India for a clean cooking energy transition. The paper endeavors to fill the void by revisiting the nature and degree of the rural energy transition for cooking in India. MethodologyUsing a multinomial logit model, the potential drivers at the individual household and group level have been identified. The group effect analysis has been conducted purposely to understand if social and cultural norms or -level factors within a village society affect the cooking energy transition of households in rural India and if that prevails over the income effect. At the sub-national level, an estimate of energy inequality has been derived by applying social choice-based Atkinson Inequality measure to examine the connection between higher energy inequality in primary fuel used for cooking and higher income inequality. In addition, the Brock-Dechert-Scheinkman (BDS) test on firewood consumption has been conducted for poor energy households from 38 districts of Bihar, one of the highest energy-poor states, to statistically examine the perceived nonlinearity in the dynamics of energy transition/fuel choice in cooking for rural households. ResultsThe analysis at the national level indicates the importance of local and cultural factors that leads to the nonlinearity in the probability of the switchover from firewood to other clean fuel options. Conclusions SummaryThe paper highlights if subsidies on modern fuel and/or other cooking alternatives alone drive the transition process and examines the validity of the energy ladder hypothesis in the case of rural cooking energy transition and the drivers of the energy transition at the national and sub-national level. Potential ImplicationsThe state-level analysis done for Bihar and across various districts corroborates this finding and provides an important direction towards local context-specific policy-making in the long term.
