Background: This study aimed to determine the profile of the antibiotic utilization in defined daily dose (DDD) units and DU 90% at surgical ward of secondary government hospital in Indonesia within six months period of study. Methods: A retrospective cross-sectional study was conducted over six months on surgical ward patients prescribed with at least one oral or parenteral antibiotic during hospitalization. Data were collected from pharmacy dispensing data and evaluated using World Health Organization (WHO) DDD. Antibiotics DDD were analysed using DU 90% segmentation in order to know the 90 percent cumulative of antibiotic consumption in surgical ward. WHO prescribing indicators were used to examine the antibiotic prescribing pattern in DU 90% segment. Results: Out of 319 antibiotic prescribed to surgical ward patients, 219 prescriptions were belonging to DU 90% segment including ceftriaxone (38.94%), moxifloxacin (16.41%), fosfomycin (14.06%), metronidazole (12.43%), and gentamicin (7.69%) in decreasing fashioned. Of the five groups of antibiotics belong to DU90%, ceftriaxone was the highest in DDD (272.5). The average number of antibiotic prescribed to patients were 1.64±0.92. The percentage of antibiotic prescribed by its generic name as well the prescribing of antibiotic from National list of essential medicines were 70.79% and 72.51%, respectively (optimal value 100%). Conclusions: Our findings on using drug utilization 90% and prescribing indicators are practical for assessing the most widely used antibiotics in ward or hospital setting and to evaluate the quality of prescribing pattern. The antibiotic utilization profile in this study can be used to provide comparative data between hospital and beneficial for future policy making to improve prescribing practice.

Background: Access, Watch, and Reserve (AWaRe) antibiotics classification was released in 2019 by the World Health Organization (WHO) to enhance antimicrobial stewardship programs in all healthcare facilities. As a result, WHO advises global action to increase the availability of antibiotics from the Access group by more than 60%.Objective: to determine antibiotics consumption for outpatients at a public tertiary hospital in Bengkulu, Sumatera-Indonesia, from 2018 to 2022, focusing on antibiotics from Access class according to the AWaRe classification from WHO and Ministry of Health Republic of Indonesia (MoHRI). Methods: This is a cross-sectional survey analyzing aggregate data on antibiotics use for outpatients at the hospital during the study period. Data on antibiotics were collected from the hospital pharmacy department, while data on patient visits were collected from the medical records department. The quantity of antibiotics used was calculated using the ATC/DDD method and expressed in DDD/1000 patient-day (PD), which was then converted into a percentage. Results: During the study period, 50-60% and 65-73% out of 14-19 antibiotic agents are from Access class according to WHO and MoHRI AWaRe classification, respectively. Quantitatively, according to the WHO and MoHRI AWaRe classification, the consumption of antibiotics from the Access class was 25-50% and 33-71% of total consumption, respectively. In addition, the segment of drug utilization 90% (DU90%) of antibiotics was dominated by antibiotics from Watch class. Conclusion: The hospital has not yet met the WHO target for antibiotic consumption from the Access class, highlighting the need for some effective efforts from Watch class to limit the usage of antibiotics.

Background: Evaluation of the use of antibiotics is the control of antimicrobial resistance. The ATC/DDD method was recommended to evaluate drug use quantitatively which can be used to compare the quantity of antibiotic use between hospitals and between countries. This research was a descriptive study with retrospective data collection cross-sectional. This research was conducted at the ICU of Universitas Sumatera Utara hospital in August-November 2022 from medical record data for the period January-December 2021 which included patient demographics, patient diagnoses, antibiotic dosage forms, routes of administration, types of antibiotics, dosage, duration of antibiotics use, and duration of treatment. Methods: Evaluation of antibiotic doses using the ATC/DDD method. The results of the study from 57 medical records that met the inclusion criteria showed that the ICU patients who used the most antibiotics were male (59.7%), the age group was >56 years (56.14%), patient diagnoses with COVID-19 (33.33%), and the dosage form was injection with the intravenous route of administration. The most frequently used antibiotics were levofloxacin (48.54%) 45.61 DDD/100 patient-days and ceftriaxone (30.06%) 28.25 DDD/100 patient-days, and the average value of Length of Stay (LOS) is 5.4 ±3.98 days with a total DDD/100 patient-days value of 93.96 (reference value of 51–67 DDD/100 patient-days). DU 90% segment namely levofloxacin (48.69%), ceftriaxone (30.16%) and meropenem (12.82%). The results of the study concluded that the use of antibiotics in ICU patients at Universitas Sumatera Utara Hospital showed a high DDD/100 patient-days rate. Therefore, it is necessary for hospital to continuously monitor and evaluated the rational use of antibiotics to prevent resistance of antibiotics.
