To test the hypothesis that the low socioeconomic population living is shanty towns in Porto Alegre presents different levels of poverty which are reflected on its health status, a cross-sectional study was designed involving 477 families living in Vila Grande Cruzeiro, Porto Alegre, Brazil. The poverty level of the families was measured by using an instrument specifically designed for poor urban populations. Children from families living in extreme poverty (poorest quartile) were found to have higher infant mortality rate, lower birth weights, more hospitalizations, and higher malnutrition rates, in addition to belonging to more numerous families. Thus, the shanty town population of Porto Alegre is not homogeneous, and priority should be given to the more vulnerable subgroups.

Resumo
ObjetivoDescrever a evolução da assistência materno-infantil, com base nas informações extraídas de dois inquéritos domiciliares realizados nos anos de 1984/85 e de 1995/ 96, na cidade de São Paulo, SP. Métodos Foram estudadas amostras probabilísticas da população entre zero e 59 meses de idade: 1.016 crianças em 1984/85 e 1.280 crianças em 1995/96. Três componentes da assistência materno-infantil foram investigados: assistência pré-natal, assistência ao parto e ao recém-nascido e assistência de puericultura. Resultados Aspectos favoráveis da evolução da assistência materno-infantil na cidade de São Paulo foram a manutenção da cobertura universal da assistência hospitalar ao parto, a expansão substancial do alojamento conjunto mãe-filho na maternidade, o aumento no número de consultas de puericultura no primeiro ano de vida e, sobretudo, a universalização da cobertura das vacinas BCG, tríplice e anti-sarampo. Aspectos desfavoráveis foram o progresso muito modesto e claramente insuficiente da assistência pré-natal, a manutenção de quase 50% de cesarianas e a não expansão da puericultura após o primeiro ano de vida. Conclusões Estimativas sobre a evolução da assistência materno-infantil no conjunto das áreas urbanas do País em período semelhante confirma e acentua o desempenho insatisfatório da assistência pré-natal na cidade de São Paulo. Comparações favoráveis a São Paulo são registradas apenas quanto a cobertura vacinal. A influência que mudanças na assistência materno-infantil podem ter exercido sobre a evolução de diferentes indicadores do estado de saúde das crianças da cidade é examinada em artigos subseqüentes.

Monteiro et al (10) estudaram a cobertura e a qualidade da assistência em puericultura no Município de São Paulo, em amostra de crianças menores de 5 anos, em 1983 e 1984. A cobertura de puericultura para crianças de 12 a 59 meses foi quase universal, de 98%; 65% das crianças realizaram a primeira consulta preventiva nos dois primeiros meses de vida.…”
Pediatric care is fundamental to preventing several diseases during the early years of childhood. Early medical care and having at least nine checkups in the first year of life are the ideal in pediatric care. It is essential to periodically assess whether or not these goals are being met. In 1994, a cross-sectional study was carried out using three-stage cluster sampling to measure coverage of pediatric care and factors associated with that care among children in the city of São Luís, in the state of Maranhão, Brazil. For data collection, a standardized questionnaire was used with the mother of the child or another person responsible for the child. The percentage of those refusing to participate or who were absent was 7%. This work analyzes data concerning 290 children between 1 and 23 months of age. The coverage estimates and 95% confidence intervals took into consideration possible effects of the study design. Cox's regression, modified for cross-sectional studies, was used to estimate adjusted relative risks. Pediatric care coverage for the population studied was 80%. The children had an average of 7.8 checkups in the first year; 44% of the children had six or fewer checkups during the first year of life, well below the recommended number. Eighty-one percent of the children were seen in the Brazilian public health care system (Sistema Unico de Saúde, SUS), and only 15% used health insurance. After adjusting for confounding factors, the factors "greater number of siblings at home" and "father with four or fewer years of schooling" were associated with not having checkups. After adjustment, family income was not associated with the number of checkups. Encouraging educational activities and perhaps also family planning programs are important strategies for increasing the coverage of pediatric care in the city of São Luís.
