Acute tonsillitis remains one of the common childhood diseases in developing countries. Prompt and appropriate treatment based on the knowledge of the causative microbiota and their antimicrobial susceptibility pattern will improve the treatment outcome and reduce time and resources spent on treatment. This study aims to determine the pattern of microbiota isolates and their susceptibility pattern. The study was a combination of the retrospective and cross-sectional method. The medical records of children treated for tonsillitis were retrieved, as well as those of children who presented with acute tonsillitis for the study. Interviewer-administered questionnaire was used to collect data, as well as document information retrieved from their medical record: presenting symptoms, treatments received. Swab sample was taken for culture and antibiotic susceptibility test. Out of the 72 swab cultures, 29 (40.3%) yielded insignificant growth of gram positive cocci. A total of nine (9) different isolates were obtained from all the significant swab cultures. Streptococcus species (13, 18.6%) and staphylococcus species (11, 15.3%) were the commonest isolates. Imipenem and levofloxacin revealed high level of susceptibility, while Ampicillin and Cefexine recorded high resistance rates. The proportion of the cultures that were positive of significant growth, the proportion of these positive isolates that were Streptococcus spp. and varied sensitivity pattern obtained underpinned the importance to advocate for culture isolates and susceptibility pattern guided treatment. These will not only an efficient approach to management of acute tonsillitis, but also a strong approach towards effective implementation of antibiotic stewardship.

Background: Peritonsillar abscess is a common pathology that presents to an otolaryngologist with poor documentation especially in developing countries. The aim of this study was to describe the clinical profile and management of peritonsillar abscess in our practice. Material and Method: This was a retrospective study of patients presenting to our Ear, Nose and Throat facility with diagnosis of peritonsillar abscess. Information obtained includes detailed demographic characteristics, clinical findings and management of the patients. All obtained data were collated, entered into computer system and analyzed using SPSS version 18.0. Results: Prevalence of peritonsillar abscess was 0.2%. There were 38.9% males and male to female ratio was 1:1.6. Duration of illness at presentation with more than 1 week was 75.9% and single episode occurred in 85.2%. Right peritonsillar abscess accounted for 66.7% and aspirated pus of more than 5 ml was recorded in 48.1%. Majority (87.0%) of the patients was referred and 42.6% of such referral is by family physician. Patients seen in ear, nose and throat outpatient clinic were 83.3%. Common symptoms at presentation were fever, sore throat, odynophagia and otalgia in 100%, 94.4%, 90.7% and 59.3% respectively. All the patients (100%) had enlarged tonsils and with deviated uvula on clinical examination followed by 98.1% exudate on the tonsils and 31.5% trismus. Test aspirate confirmed peritonsillar abscess in 68.5% and all revealed growth of bacteria. Common isolated bacteria were 35.2% Streptococcus species and 11.1% Staphyloccocus aureus. Isolated Streptococcus species and Staphylococcus aureus had 100% sensitivity to cefuroxime, gentamicin, azithromycin, ceftazidine, Amoxicillin and Streptomycin. Prior prehospital treatment was received by 81.5% patients. Hospital treatments were surgical intervention and medical treatment in 90.7% and 100% respectively. Recurrences occurred in 3.7% during followed up. Conclusion: Peritonsillar abscess are common disorder with female preponderance in this study. The prevalence recorded in this study was 0.2%. It usually mimic other causes of fever hence eye index of suspicion is essential. Surgical treatment remains the effective mode of management in our study.

Tonsillitis is the inflammation of the tonsils. The tonsils are two small glands on either side of the throat. The tonsils play a key role in helping the body to protect against infection. This is especially in young children, whose immune system is still developing. The tonsils act as a barrier, trapping an infection and stopping it spreading to other parts of the body. In Ayurveda tonsillitis is correlated with Tundikeri, the disease of Talu (palate) and Kantha (throat), both of which are the Avaranas (establishment) of Mukharoga (diseases of mouth). Although no specific formulation for Tundikeri is given and surgical treatment is described for the disease, many formulations for Mukharogas (mouth disorders) are effective in Tundikeri. Vyaghri haritaki lehya is described by Bhaishajya Ratnavali and Chakrvadatta. It is also included in Bhava Prakasha, Yoga Ratnakara, Gada Nigraha, Vanga Sena samhita and Bharat Bhaishajya Ratnakar. A case study of 20 children on chronic tonsillitis done using Ayurveda medicine to improve general health.. Internal administration of Vyaghri haritaki lehya, the dose was 5gm for children between 3 to 6 years and 10gm for children between 7 to 12 years with lukewarm water as adjuvant, for a period of 60 days given. Follow up done after a period of 30 days.
