Endoscopic peroneal retinaculum reconstruction

Abstract: This is a technical note on endoscopic peroneal retinaculum reconstruction in the management of peroneal tendons instability. The surgical trauma of open peroneal retinaculum reconstruction can be minimized with endoscopic approach. The author describes the surgical principle and technique of this endoscopic approach.

“…It also allows to treat several disorders: adhesiolysis, synovectomy, resection of a fibular or calcaneal exostosis, debridement or suturing of partial tendon rupture, groove deepening. Lui described an endoscopic reconstruction of the peroneal retinaculum [10]. Tendoscopy has demonstrated several advantages, such as less pain, outpatient treatment, functional after-treatment, and rapid resumption of work and sport activities.…”

“…However, if patients have a concave fibula, fibular groove deepening is not reasonable. On the other hand, Guillo and Calder, 5 Lui, 6 and Miyamoto et al 7 reported tendoscopic repair of the retinaculum for peroneal tendon subluxation or dislocation. However, these surgical techniques were only single-row methods.…”

“…64 Recently, an endoscopic technique for the anatomic repair of the SPR has been described. 69 Several authors have described procedures to augment or reinforce an attenuated SPR with tissue transfer consisting of either tendon or periosteal flaps. Ellis-Jones 70 first described restraining the peroneal tendons with a strip of Achilles tendon anchored through a drill hole in the fibula.…”
