67ОБЗОРЫ ТО М L X I I В Ы П УС К 6 / 2 0 1 3 В настоящее время не существует консенсуса о клеточ-ной и молекулярной природе генитального эндометриоза и, несмотря на большие достижения в понимании этиологии и патогенеза, клиницисты не имеют единой тактики лечения заболевания. Одним из распространенных клинических про-явлений эндометриоза является нарушение репродуктивной функции. Известно, что частота бесплодия при всех локали-зациях генитального эндометриоза примерно в 3-4 раза пре-вышает частоту бесплодия в популяции, а частота самопро-извольного прерывания беременности (чаще в 1 триместре) колеблется от 10 до 50 % [1]. По некоторым данным [34], до 50 % женщин с эндометриозом бесплодны. Однако данные литературы, посвященные этой теме, носят противоречивый характер. бесплодие и эндометриозБесплодие определяется как отсутствие наступления бе-ременности через 1 год регулярной половой жизни без кон-трацепции. У здоровых женщин вероятность наступления беременности в каждом отдельном менструальном цикле со-ставляет 15-20 %, тогда как у женщин с генитальным эндо-метриозом без терапии эта вероятность снижается до 2-10 % [43,74]. Существует множество систем стадирования и оцен-ки степени эндометриоза [34], но не каждый метод предска-зывает исходы, относящие к фертильности в каждом цикле. Факторы, влияющие на фертильность при эндометриозе, сгруппированы в парадигму оценки риска бесплодия -ин-декс фертильности при эндометриозе (EFI) [4]. EFI предска-зывает частоту наступления беременности у пациенток с хи-рургически подтвержденным эндометриозом, основываясь на степенях пересмотренной классификации американского общества фертильности (r-AFS), функциональной оценке ма-точных труб и яичников, длительности бесплодия и анамне-стических данных о беременностях.Связь между эндометриозом и бесплодием была доказа-на более 20 лет назад на моделях животных [9,25,40,68]. Негативное влияние эндометриоза на фертильность под-тверждается опытом применения инсеминации донорской спермой в парах с мужским фактором бесплодия. Частота на-ступления беременности при применении донорской спермы варьировала в зависимости от возраста и составляла от 54 % до 75 % со средней частотой ежемесячного наступления бе-ременности (MFR) 0,05-0,13 (5-13 % в месяц) при примене-нии искусственной инсеминации [34]. Сообщалось о частоте MFR 0,12 у здоровых женщин по сравнению с показателем 0,036 у женщин с эндометриозом минимальной степени рас-пространенности [34]. Objective: Aromatase is the key enzyme for the conversion of C19 steroids into estrogen in certain human tissues. We studied to evaluate the aromatase expression in eutopic endometirum and endometriotic lesion and its relationship to clinical and laboratory parameters. Methods: The study included 78 cases of endometriotic lesion and 14 cases of eutopic endometrium and 30 cases of normal uterine endometrium obtained through laparoscopic surgery and curettage. The frozen tissue specimens were examined by immunohistochemistry using aromatase. Clinical symptoms, laboratory findings, and operative findings were analyzed and compared in according to aromatase expression. Results: We observed positive immunohistochemical expression for aromatase in endometriotic lesion from 46/78 patients (59.0%). Aromatase expression was elevated in comparison to eutopic endometrium (5/14 patients, P=0.032) and the difference was more pronounced when eutopic endometriums from patients with endometriosis were compared with those of healthy controls (2/30 patients, P<0.001). Aromatase-positive patients had more moderate-to-severe chronic pelvic pain, higher CA-125 level significantly. Also in operative findings, severe grade endometriosis, bilateral endometriomas, and associated leiomyoma and adenomyosis were more frequent in aromatase positive patients. High values of white blood cell count, erythrocyte sedimentation rate, CA 19-9 were more frequent in aromatase positive patients notwithstanding insignificant differences. Conclusion: Unopposed local biosynthesis of estrogens by increased expression of aromatase in eutopic endometrium and endometrial tissue could be involved in the development or maintenance of endometriosis and other uterine estrogen-triggered diseases. Our findings suggest increased expression of aromatase may be related with severity, activity, and chronic pelvic pain in patients with endometriosis. Abstract: These results suggest that alteration in the production of cytokines in the PF, especially IL-6, besides contributing to the endometriosis and its evolution, probably increases embryotoxicity. However, no correlation was found between the latter and associated infertility.
