Periodontitis is a dysbiosis‐driven inflammatory disease affecting the tooth‐supporting tissues, characterized by their progressive resorption, which can ultimately lead to tooth loss. A step‐wise therapeutic approach is employed for periodontitis. After an initial behavioral and non‐surgical phase, intra‐bony or furcation defects may be amenable to regenerative procedures. This review discusses the regenerative technologies employed for periodontal regeneration, highlighting the current limitations and future research areas. The search, performed on the MEDLINE database, has identified the available biomaterials, including biologicals (autologous platelet concentrates, hydrogels), bone grafts (pure or putty), and membranes. Biologicals and bone grafts have been critically analyzed in terms of composition, mechanism of action, and clinical applications. Although a certain degree of periodontal regeneration is predictable in intra‐bony and class II furcation defects, complete defect closure is hardly achieved. Moreover, treating class III furcation defects remains challenging. The key properties required for functional regeneration are discussed, and none of the commercially available biomaterials possess all the ideal characteristics. Therefore, research is needed to promote the advancement of more effective and targeted regenerative therapies for periodontitis. Lastly, improving the design and reporting of clinical studies is suggested by strictly adhering to the Consolidated Standards of Reporting Trials (CONSORT) 2010 statement.

Objetivo: Esta revisión sistemática tuvo como objetivo evaluar los efectos del ácido hialurónico (AH) solo o en combinación con cualquier sustituto óseo para el tratamiento de los defectos intraóseos (DIO). Material y métodos: Se realizaron búsquedas en seis bases de datos hasta abril de 2022 para encontrar ensayos clínicos aleatorizados que compararán los efectos clínicos del desbridamiento con colgajo abierto (DCA)+AH frente al DCA solo (primer grupo) o DCA+AH + sustitutos óseos frente a DCA + sustitutos óseos (segundo grupo) en el tratamiento de los DIO con un seguimiento de al menos 3 meses. Se utilizaron modelos de efectos aleatorios de diferencias medias para determinar la ganancia del nivel de inserción clínica (NIC), la reducción de la profundidad de sondaje (PS) y el relleno óseo radiográfico (ROR). Resultados: De los 276 estudios identificados, 6 se incluyeron en la síntesis cualitativa y 5 en los metaanálisis. Los metaanálisis del primer grupo mostraron diferencias estadísticamente significativas en la ganancia del NIC (diferencia media [DM]: 1,00; intervalo de confianza [IC] del 95%: 0,65-1,35; n = 2) y la reducción de la PS (DM: 0,76; IC del 95%: 0,34-1,17; n = 2) a favor de AH+DCA a los 6 meses. Sin embargo, en el segundo grupo, los metaanálisis no demostraron un efecto adicional del AH en asociación con el sustituto óseo ni para la ganancia del NIC (DM: 0,57; IC del 95%: -0,30-1,43; n = 2) ni para la reducción de la PS (DM: 1,05; IC del 95%: -0,38-2,47; n = 2), pero sí mostraron diferencias significativas para el ROR (DM: 0,57; IC del 95%: 0,15-0,99; n = 2) a los 12 meses. Conclusiones: En comparación con el DCA solo, la aplicación local de AH en el tratamiento de los DIO proporcionó una ganancia significativa del NIC y una reducción de la PS a los 6 meses. Sin embargo, su combinación con sustitutos óseos no mostró diferencias estadísticamente significativas a los 12 meses. Importancia clínica: El uso de DCA+AH mejora el NIC y la PS en el tratamiento de los DIO en comparación con DCA sólo tras 6 meses de seguimiento. Estos resultados no se mantienen a los 12 meses.
