Efficacy and safety of Tripterygium wilfordii Hook F for CKD in Mainland China: A systematic review and meta‐analysis

Abstract: Tripterygium wilfordii Hook F (TwHF) is a promising Chinese traditional medicine used to significantly reduce proteinuria and improve renal function. However, its efficacy and safety in treatment of chronic kidney disease need to be further explored in order to promote its application in clinics. This review compared the efficacy and safety of TwHF with the placebo, conventional Western medicine and other immunosuppressive medicine in a range of kidney disorders. One hundred three randomized controlled trials … Show more

“…A meta-analysis of RCTs carried out in patients with stage 4 diabetic CKD showed that TwHF combined with an ARB was more effective than an ARB alone when considering 24 h proteinuria and serum albumin, but with an increase in AE frequency of 8% [32]. A recent review indicated that TwHF-based therapies for CKD in mainland China were more effective in improving proteinuria, blood creatinine levels, and blood urea nitrogen levels than conventional Western medicine alone or other immunosuppressive regimens [33]. Meta-analyses highlighted that high-quality trials are still needed to confirm treatment efficacy [33,34].…”

Meta-analyses highlighted that high-quality trials are still needed to confirm treatment efficacy [33,34]. A preliminary Chinese study showed that TwHF combined with an ARB decreased proteinuria by about 30% throughout 6 BioMed Research International the treatment period, while the ARB alone had little-to-no effect on proteinuria [27].…”

“…This indicates that the therapeutic dose of TwHF is close to its toxic dose, resulting in unavoidable occurrence of AEs. However, its safety can be controlled within acceptable limits (Wang et al, 2018; Zhou et al, 2018). Hence, the China State Food and Drug Administration issued a drug warning on TwHF preparations in April 2012 to restrict its clinical application (The State Food and Drug Administration, 2012).…”

“…The tripterygium glycoside tablets have been used as the effective drug for malignancy such as Crohn's disease and HIV/ AIDS due to its various pharmacological activities [3,4]. Moreover, tripterygium glycoside tablets have also been used in kidney disease because it can significantly reduce proteinuria and preserve renal function [5]. Tripterygium glycoside tablets are showing more therapeutic potential [5].…”

Tripterygium glycoside tablets are showing more therapeutic potential [5]. But it should be noted that tripterygium glycoside tablets have a high incidence of adverse reactions, including gastrointestinal discomfort, diarrhea, reproductive toxicity, hematological disorders, and liver damage [6].…”
