Effect of percutaneous transluminal angioplasty on tissue oxygenation in ischemic diabetic feet

Abstract: Percutaneous transluminal angioplasty (PTA) has been performed as an alternative to bypass surgery for improving tissue oxygenation in ischemic diabetic feet because the former is less invasive than the latter. The purpose of this study was to evaluate the effect of PTA on tissue oxygenation in ischemic diabetic feet. This study included 29 ischemic diabetic feet, as determined by a transcutaneous oxygen pressure (TcPO(2) )<30 mmHg. The PTA was carried out in 29 limbs. The PTA procedure was considered successf… Show more

“…After revascularization, TcPO 2 progressively improved in the successfully revascularized group, with a TcPO 2 >30 mm Hg in 38.5% of patients 1 week after PTA and reaching its peak value in the 4 th week by increasing to 75%. These results have been confirmed by others [16,17]. Pardo et al [16] performed a prospective study with 151 patients, 64 of which underwent revascularization via PTA, and then evaluated TcPO 2 pre-and postoperative and compared with ankleÀbrachial index (ABI).…”

“…Additionally, TcPO 2 could be measured in all patients, and the ABI could not be measured in 25.4% pretreatment and in 17.91% post treatment. Kim et al [17] reported on 29 diabetic patients with Fig. 1 -Angiosomes of the foot.…”

“…In our previous study on 29 limbs of 23 patients, the TcPO 2 reached its peak level in the 4th week after PTA, and the TcPO 2 was lower than the peak level in the 5th and 6th weeks after PTA. However, the results of the current study indicated that the TcPO 2 continued to increase throughout the 6th week.…”

“…14 Another reason for the delay in reaching the peak level of tissue oxygenation may be transient oedema of the lower limb after PTA. [15][16][17] In our previous study on 29 limbs of 23 patients, the TcPO 2 reached its peak level in the 4th week after PTA, 18 and the TcPO 2 was lower than the peak level in the 5th and 6th weeks after PTA. However, the results of the current study indicated that the TcPO 2 continued to increase throughout the 6th week.…”

“…In cases of chronic wounds, wound healing cannot often be achieved successfully because of the involvement of multiple factors (14, 15). One of main contributing factors for the non- or delayed healing wound is decreased function of the key cells for the wound healing including fibroblasts and keratinocytes.…”
