Objective: In view of the high degree of complexity that the polytrauma patient represents to the multiprofessional team in the elaboration and execution of their care plan in the intensive care unit (ICU), combined with the lack of evidence on the subject, this study suggests a model of early physiotherapeutic assistance to critical polytrauma patients based on the clinical experience of recent years. Method: The model was elaborated based on the practices verified in the records of 6,388 physiotherapy sessions performed in 198 patients hospitalized between December 2009 and September 2011 in polytraumas-specialized ICU. The activities/care were inserted in the model after approved in discussion with the multiprofessional team. All patients enrolled were aged 18 years or older and were victims of severe trauma according to the Injury Severity Score (ISS). Results: The proposed model was structured in such a way that the physical therapy activities/ care were organized according to the injured body region (traumatic brain injury, face fractures, spine fractures, thoracic trauma, abdominal trauma, pelvic fracture and extremities fractures). The routine of the ICU encouraged daily discussions with the medical team to know the particularities of each clinical case, to establish therapeutic goal and to design the rehabilitation program. Conclusion:The proposed model became routine and consolidated the physiotherapeutic action in the respective care unit. The physiotherapy team started working 24 hours a day. The model made possible the standardization of physical therapy assistance and greater safety for the severe polytrauma patient.

RESUMOOBJETIVOS: Avaliar os níveis álgicos de pacientes críticos, antes, durante e após uma intervenção de fisioterapia respiratória. MÉTODOS: Estudo piloto realizado na Unidade de Terapia Intensiva de um hospital escola. Foram selecionados pacientes adultos entre 18 e 60 anos, sedados e em ventilação mecânica invasiva. Foram levantados dados sociodemográficos, clínicos e hemodinâmicos e aplicada a escala Behavioural Pain Scale para avaliar a dor em pacientes incapazes de serem avaliados pelo auto relato. O protocolo de fisioterapia consistiu em manobras de vibrocompressão manual em ambos os hemitórax e aspiração intratraqueal. As variáveis hemodinâmicas e a escala de dor foram analisadas em três momentos: imediatamente antes, no quinto minuto de intervenção e imediatamente após a sessão de fisioterapia. Foi aplicada estatística com teste Shapiro-Wilk e análise de variâncias (ANOVA). O nível de significância foi p ≤ 0,05. RESULTADOS: A amostra foi composta por 22 pacientes, a maioria do sexo masculino, com média de idade de 55±23,8 anos. A doença mais frequente foi a pneumonia nosocomial. Não houve diferenças significativas nas variáveis hemodinâmicas e na avaliação da dor (p = 0,78) dos pacientes críticos em nenhum dos momentos avaliados. CONCLUSÕES: Nesta amostra de pacientes críticos, internados em unidade de terapia intensiva e em ventilação mecânica invasiva, não foram detectadas alterações nos níveis de dor durante ou após uma intervenção de fisioterapia respiratória.DESCRITORES: medição da dor; cuidados críticos; unidades de terapia intensiva; respiração artificial; especialidade fisioterapia; modalidades de fisioterapia. ABSTRACTAIMS: To evaluate pain levels of critically ill patients before, during, and after a chest physical therapy intervention. METHODS: Pilot study conducted at the intensive care unit (ICU) of a teaching hospital. Sedated and mechanically ventilated adult patients aged 18 to 60 years were selected. Sociodemographic, clinical, and hemodynamic data were analyzed, and the Behavioral Pain Scale (BPS) scale was used to assess pain in patients unable to be evaluated by self-report questionnaires. The physical therapy protocol consisted of manual vibrocompression maneuvers in both hemithoraces and of intratracheal aspiration. The hemodynamic variables and the BPS were analyzed in three periods: immediately before, in the fifth minute of intervention, and immediately after physical therapy. The Shapiro-Wilk test and ANOVA were used for the statistical analyses. The level of significance was p ≤ 0.05. RESULTS: The sample consisted of 22 patients, most of them male, aged 55±23.8 years. Nosocomial pneumonia the most prevalent pathology. There were no significant differences in hemodynamic variables and in pain assessment (p = 0.78) of the critically ill patients in any of the evaluated periods. RESUMO A prática de enfermagem em unidades de terapia intensiva demanda conhecimentos científicos atualizados sobre o neurointensivismo. Este estudo objetivou caracterizar o conhecimento da equipe de enfermagem sobre o neurointensivismo, bem como identificar se esse conhecimento se diferencia entre os profissionais que participam e os que não participam de atividades educativas pertinentes a esta temática. O estudo consiste de uma pesquisa quantitativa realizada em outubro de 2008, em duas unidades de terapia intensiva para adultos de hospitais paranaenses. Na pesquisa utilizou-se um instrumento com dados demográficos e profissionais dos entrevistados e oito questões pertinentes ao conhecimento acerca do neurointensivismo. Realizou-se análise estatística simples e inferencial (Teste Exato de Fisher ou Yates Corrigido) com o programa EpiInfo 3.5.2. Em sua maioria os participantes eram do sexo feminino (82,9%) e tinham idade de até 40 anos (89,3%). Os enfermeiros apresentaram mais acertos (média=6) do que técnicos e/ou auxiliares de enfermagem (média=3). Apenas 15 (31,9%) profissionais participaram de atividades educativas sobre neurointensivismo, e somente os cuidados de enfermagem referentes à manipulação do paciente com monitoração de pressão intracraniana (Questão 3) se associaram à realização de práticas educativas (p=0,0485). Concluiu-se ser necessário aprimoramento técnico e científico da equipe de enfermagem, para interpretação dos parâmetros monitorados e adoção de condutas seguras no manejo do doente neurocrítico.
