Drug-Induced Liver Injury

Abstract: Context Drug-induced liver injury (DILI) represents a diverse set of responses following exposure to any manufactured or naturally occurring chemical compound. Drug-induced liver injury is of major concern owing to the ever increasing number of compounds introduced into the market for treatment of various diseases as well as the increasing popularity of herbals, which lend themselves to self-medication but are not rigorously regulated. Objecti… Show more

“…Vice versa, atypical metabolism of a drug which leads to formation of toxic metabolites, generally does not cause systemic allergic manifestations and it has a long or variable latency period and frequently a late response to a repeated provocation [11,12,13]. The most common causes of idiosyncratic damage are amoxicillinclavulanate, nitrofurantoin, co-trimoxazole, ciprofloxacin, isoniazid, tyrosine kinase inhibitors [14].…”

“…There are no specific laboratory tests, histological presentations, or clinical signs and symptoms enabling the diagnosis of DILD. Signs and symptoms vary with the drug, host, and severity of injury [20]. Some situations where the probability of the existence of DILD is likely are summarized in Table 3 [18].…”

“…In Europe and USA, amoxicillin-clavulanate is the most common, in contrast to antituberculosis drugs in India. [52][53][54][55][56][57] Idiosyncratic DILI is not dose dependent per se, however, most cases occur at a dose of more than 50 mg DILIs. Drugs that undergo over 50% hepatic metabolism are also more likely to lead to hepatotoxicity.…”
