Foram investigados a prevalência e os fatores de risco da leptospirose bovina no Estado do Maranhão. O Estado foi dividido em quatro circuitos amostrais com base em parâmetros de produção distintos que variam conforme os diferentes sistemas de produção, as práticas de manejo, a finalidade de exploração, o tamanho médio dos rebanhos e os sistemas de comercialização. Objetivou-se estudar as características epidemiológicas da leptospirose bovina no Estado do Maranhão, de modo a determinar a prevalência em bovinos e em rebanhos, detectar as sorovariedades de Leptospira spp. presentes, identificar os fatores de risco eventualmente associados à leptospirose em bovinos e diferenciar os circuitos pecuários entre si no que se refere à prevalência de leptospirose. A pesquisa foi realizada em 136 propriedades rurais pertencentes ao circuito I, no qual 841 fêmeas bovinas com idade igual ou superior a 24 meses foram analisadas; 238 do circuito II, com 2.582 fêmeas analisadas; 122 do circuito III, com 869 fêmeas analisadas; e 77 do circuito IV, com 540 fêmeas analisadas; no total, 573 propriedades e 4.832 fêmeas foram estudadas. A presença de anticorpos contra Leptospira spp. foi verificada pela técnica de soroaglutinação microscópica (SAM). Das 4.832 fêmeas bovinas analisadas, 1.904 (35,94%; IC 95% = 33,01% - 38,98%) foram reagentes. Das 573 propriedades analisadas, 380 (64,81%; IC 95% = 61,10% - 68,35%) foram consideradas positivas. As sorovariedades Hardjo e Wolffi foram as mais frequentes em todo o Estado. O circuito III foi o que apresentou menor prevalência de leptospirose em todas as comparações. As variáveis identificadas como fatores de risco de leptospirose foram: presença de equinos (p = 0,000), presença de capivaras (p = 0,034) e rebanhos bovinos com 32 ou mais fêmeas adultas (p = 0,002).

OBJECTIVE:To identify areas at risk of dengue transmission by means of cluster analysis. METHODS:A cluster analysis in which the primary analysis units were the 48 districts of the municipality of Niterói, Southeastern Brazil, was conducted. The districts were grouped into six strata according to sociodemographic conditions, using the k-means cluster analysis method. After defi ning the strata, the incidence of dengue was calculated for each stratum in relation to four different periods : 1998 -2000; 2001; 2002; 2003 -2006. RESULTS:The analysis on the incidence showed that the rates for the last three study periods were greatest in the stratum 2.1, which had the worst sanitation infrastructure conditions and high population increases, and in stratum 3.1, which had the highest percentage of shantytowns. Stratum 1.2 presented the lowest incidence and the best sanitation and income indicators, along with small increases in population and a low proportion of shantytowns. The incidence rates in 2001 and 2002 were high in most strata except for stratum 1.2, which had the districts with the least heterogeneity in relation to the indicators used. In 2001, the strata presented high rates of incidence when group immunity had supposedly become established for serotype I, thus expressing the transmission strength of this agent. CONCLUSIONS:The cluster analysis technique made it possible to recognize priority areas. It indicated areas where the dengue control and surveillance actions needed to be improved, along with structural improvements that infl uenced the living conditions and health of the municipality's population.

OBJECTIVE:To analyze the spatial and seasonal distribution of leptospirosis and identify possible ecological and social components of its transmission. METHODS:A total of 2,490 cases registered in each district of the municipality of São Paulo, Southeastern Brazil, between 1998 and 2006, were georeferenced. The data were obtained from the Notifi able Diseases Information System. Thematic maps were made, showing the variables of incidence rate, lethality rate, literacy rate, average monthly income, number of residents per household, water supply and sewage system. To identify spatial patterns (dispersed, clustered or random), these variables were analyzed using the global and local Moran indices. Spearman's correlation coeffi cient was used to test associations between variables with clustered spatial patterns. RESULTS:Clustered spatial patterns were observed for the variables of leptospirosis incidence, literacy rate, average monthly income, number of residents per household, water supply and sewage system. There were 773 notifi ed cases in the dry season and 1,717 cases in the rainy season. The incidence and lethality rates correlated with the population's socioeconomic conditions, independently of the period. CONCLUSIONS:Leptospirosis is spread throughout the municipality of São Paulo, and its incidence increases during the rainy season. In the dry season, the localities where cases appear coincide with the areas of poorest housing conditions. In the rainy season, it also increases in other districts, probably due to the proximity of rivers and streams.
