La obesidad infanto-juvenil es un problema de salud pública mundial con una alta prevalencia de dislipidemia. El objetivo de esta investigación fue establecer las variaciones del perfil lipídico e índices aterogénicos en adolescentes obesos. Estudio observacional, descriptivo, transversal, con un muestreo probabilístico de 225 adolescentes obesos y un grupo control de 100 adolescentes normopesos. La evaluación antropométrica se realizó con las curvas de la Organización Mundial de la Salud (OMS), en la evaluación bioquímica: se determinaron colesterol total (CT), colesterol de las lipoproteínas de alta densidad (HDL-C) y triglicéridos (TG) mediante metodología analítica seca y el colesterol de las lipoproteínas de baja densidad (LDL-C) con la Fórmula de Friedewald: CT - (HDL-C + TG/5) en mg/dl y también se aplicaron los índices aterogénicos: no-HDL-C, CT/HDL-C, TG/HDL-C, LDL/HDL-C. En los resultados predominó la obesidad en el género femenino a la edad de 13,4±1,8 años, pero las variables antropométricas en el género masculino tuvieron una diferencia significativa en peso, índice de masa corporal (IMC) y circunferencia de cintura, el 84% de los adolescentes obesos presentaron dislipidemia con tríada lipídica: hipertrigliceridemia, hiperlipoproteinemia de alta densidad e hiperlipoproteinemia de baja densidad, con una diferencia significativa p<0,001 en los índices aterogénicos con riesgos de enfermedades cardiovasculares: TG/HDL-C 12,77±1,3, no-HDL-C 249,4 mg/dl, CT/HDL-C 13,34±1,6 y LDL-C/HDL-C 9,17±1,1. Se concluye que los adolescentes obesos presentaron dislipidemias, tríada lipídica e índices aterogénicos elevados, considerados en mayor riesgo de enfermedades cardiovasculares en la misma adolescencia y en la adultez.
