BackgroundCognitive dysfunction in fibromyalgia (FM) encompasses objective cognitive difficulties, as measured in neuropsychological tests, and self-reported cognitive complaints. Although it has been suggested that FM patients display problems in working memory, the data are inconsistent, and the overall working memory status of the patients is unclear. It is also not clear whether the working memory problems are related to cognitive complaints or how the dyscognition is affected by the characteristic clinical symptoms of FM.MethodsTo clarify these aspects, we explored the neuropsychological performance for different components of working memory and the subjective self-perception of cognitive status in a sample of 38 women with FM. They were compared with a matched group of 32 healthy women.ResultsOur findings suggested that the FM patients do not differ from healthy controls in their overall working memory functioning. Only a poor performance was found in a single task of visuospatial working memory, mediated by the presence of depressive symptoms, fatigue and pain. The FM patients also displayed a higher level of perception of cognitive difficulties than healthy controls, and this difference was mediated by depression and fatigue. Furthermore, cognitive complaints in FM patients were only associated with a lower verbal WM capacity.DiscussionFM patients have a subtle specific impairment in their working memory functioning, as well as elevated concern about their cognitive status. These findings suggest a disconnection between neuropsychological performance and subjective complaints. In FM patients, clinical variables such as pain, fatigue, and depression play an important role in dyscognition, as assessed by both objective and subjective measures, and should be taken into account in future research.

El síndrome de fibromialgia (sf) es una patología que genera gran dificultad en los movimientos, fatiga y dolor en partes del cuerpo, afectando severamente la calidad de vida. El objetivo de este estudio fue determinar los indicadores de depresión, ansiedad, apoyo interpersonal, dolor autopercibido y su relación, en una muestra de pacientes con sf en Colombia. A 100 pacientes se les aplicaron las siguientes pruebas: Inventario de Ansiedad Rasgo Estado, Escala de depresión de Zung, Cuestionario para evaluación del apoyo interpersonal, y la Escala analógica del dolor. Se reportaron niveles moderados de depresión y ansiedad, dificultades específicas en el apoyo social y niveles altos en dolor autopercibido. Las puntuaciones en la escala de depresión se asociaron positivamente con la ansiedad estado y rasgo, pero se asociaron negativamente con el apoyo social. En conclusión, la salud mental, el apoyo interpersonal y el dolor autopercibido se encuentran afectados en los pacientes. Se requiere ajustar los protocolos de atención clínica con el propósito de conservar y potenciar la salud integral en los afectados.

Objective: Patients with fibromyalgia syndrome (FMS) have mild cognitive dysfunction. Higher serum uric acid (UA) levels in some diseases are reported to be associated with cognitive impairment. Mean platelet volume (MPV) is an indicator of platelet activation. Activated platelets may play a role in neuroinflammation and cognitive dysfunction. The purpose of this study was to compare cognitive functions of FMS patients and controls and investigate the relation of serum UA and MPV levels with cognitive functions. Method: Fifty-four patients with FMS and 33 healthy controls were enrolled,retrospectively. Evaluations were performed with Visual Analog Scale (VAS), Fibromyalgia Impact Questionnaire (FIQ), Beck Depression Inventory(BDI) and to asses phychological status and cognitive impairment Mini Mental State Examination(MMSE) was used in both groups. Results: Mean VAS and sleep quality of FMS group were significantly higher than the control group (p<0.05). Mean MMSE score was statistically significantly lower in the FMS group (p=0.0001). There was not statistically significant correlation between serum UA levels and VAS pain/sleep quality, FIQ, BDI and MMSE scores (p>0.05). No significant correlation was found between serum MPV levels and VAS pain/ sleep quality, FIQ, BDI and MMSE scores (p>0.05) but declined cognitive functions were determined in the FMS group when compared with the control group. Conclusion: Declined cognitive functions were determined in the FMS the FMS group when compared with the control group. Although we did not determine an association of serum uric acid and MPV levels with cognitive functions, larger prospective studies with longer follow up periods are warranted to elucidate the role of these biomarkers in FMS and to determine the factors affecting cognitive functions.
