Multiple biopsies under endoscopic guidance have shown that chronic gastritis is frequently patchy, involving the antrum and the lesser curve and sparing often the greater curve. Anacidity and parietal cell antibodies which are thought to indicate the presence of diffuse atrophic changes may be found in patchy gastritis. Endoscopic biopsies may be superficial and unable to diagnose mucosal hyperplasia; they are more costly than blind biopsy techniques, and they are unpractical for survey studies involving large numbers of patients. In spite of these shortcomings, multiple endoscopic biopsy is the only reliable procedure for the assessment of chronic gastritis. Unless diffuse lesions are suspected, as in pernicious anemia patients, blind biopsy should not be considered for diagnostic purposes.
