Introduction: The spleen is a vital lymphoid soft organ that demands constant attention from the clinical point of view. It is multi-dimensional organs that enlarge in all dimensions in disease conditions. The detection of the spleen by palpation on physical examination is not an approval of an enlarged spleen because an unenlarged spleen may palpable. Therefore, this study aimed to assess the Morphometry of spleen dimensions and its correlates among individuals living in Arba Minch Town by sonographic examinations.Methods and Materials: Community-based cross-sectional study was conducted in Arba Minch town from February 1 to March 30, 2020. A multi-stage systematic and simple random sampling technique was applied to select study participants. Data was entered into Epi-Data version 3.1software and exported to SPSS version 20 software for analysis.Result: A total of 708 individuals, 390 (55.1%) males and 318 (44.9 %) females were included in the current study. The mean splenic length, width, thickness and volume were 10.24 cm, 4.79 cm, 3.93 cm, and 109.34 cm³, respectively. The mean spleen length, width, thickness and volumes among males were 10.64 cm, 4.92 cm, 4.05 cm and 119.81 cm³ and among females were 9.75 cm, 4.63 cm, 3.78 cm and 96.50 cm³ respectively. Significant negative correlations were found between age and spleen dimensions. Significant positive correlations were observed between spleen dimensions and height, weight, Body Mass Index, and Body Surface area. But, the correlation between width and body mass index was non-significant (p-value > 0.05).Conclusion and Recommendations: The spleen dimensions were higher in males than females and have a significant positive correlation with height, weight, Body Mass Index and Body Surface Area except for the width. Further studies in different parts of the country have to be done to have normative data on the dimension of Spleen in Ethiopia.

Background: Studies in small groups of patients indicated that splenic volume (SV) may be decreased in patients with celiac disease (CD), refractory CD (RCD) type II and enteropathy-associated T-cell lymphoma (EATL). Objective: The objective of this article is to evaluate SV in a large cohort of uncomplicated CD, RCD II and EATL patients and healthy controls. Methods: The retrospective cohort consisted of 77 uncomplicated CD (of whom 39 in remission), 29 RCD II, 24 EATL and 12 patients with both RCD II and EATL. The control group included 149 healthy living kidney donors. SV was determined on computed tomography. Results: The median SV in the uncomplicated CD group was significantly larger than in controls (202 cm 3 (interquartile range (IQR): 154-275) versus 183 cm 3 (IQR: 140-232), p ¼ 0.02). After correction for body surface area, age and gender, the ratio of SV in uncomplicated CD versus controls was 1.28 (95% confidence interval: 1.20-1.36; p < 0.001). The median SV in RCD II patients (118 cm 3 (IQR 83-181)) was smaller than the median SV in the control group (p < 0.001). Conclusion: This study demonstrates large inter-individual variation in SV. SV is enlarged in uncomplicated CD. The small SV in RCD II may be of clinical relevance considering the immune-compromised status of these patients.

urpose. The paper is based on a seven-year surveillance of the patient showing ectopic adrenocorticotropic hormone (ACTH) syndrome. Materials and methods. A female 49-year old patient underwent pancreaticoduodenectomy for pancreatic head neuroendocrine tumor (NET) with synchronous liver metastases in 2009. Over time the metastatic lesions were gradually enlarging and conventional transarterial chemoembolization (cTACE) was performed twice: on December 2012 and on March 2014 in "on demand" regimen. Since 2013 the patient developed clinical signs suspicious for hypercortisolism but the elevated serum cortisol and ACTH levels were registered only on May 2015. ACTH production was proven to be ectopic by dexamethasone suppression test and it was a quite rare finding in a pancreatic NET. At the same time the spleen volume on MRI was evaluated as 126 cm3 which was almost two times lower than in 2009 (231 cm3).Results. With regard to occasional papers which mentioned cTACE as a method for hypercortisolism abatement the third cTACE was performed on June 2015. It resulted in tumor destruction, normalization of cortisol and ACTH levels and restoration of the spleen size, ongoing up to the date (12 months after the procedure). The retrospective MRI-based spleen volumetry demonstrated reduction of the spleen volume overall, and its increase in a period 1.5-12.0 months after each cTACE (r2=0.5, p<0.006). The spleen volume also correlated with ACTH levels (p<0.0001). Thought a remarkable spleen weight decrease after prolonged dexamethasone treatment has been shown on rodent models, PubMed search revealed no previous clinical reports focusing on splenic volume evolution depending on glucocorticoid levels.Conclusions. The presented case makes us hypothesize that the consistent pattern similar to that in the animal studies is also present in humans and, thus, microsplenia in cancer patients may serve as an indirect visualization sign of the ectopic ACTH production.Keywords: spleen, neuroendocrine tumors, chemoembolization, ACTH-ectopic syndrome, MRI, volumetry.
