Myrmeleon sp. is typical of insect larva which has been used in Indonesia for diabetes treatment. However, there is no sufficient scientific report explaining the bioactive compounds in this insect. Based on our preliminary research, this insect contained metformin, i.e. one of bioactive compounds for the treatment of type-2 diabetes. Therefore, this study is focused on the development of separation technique using high-performance liquid chromatography (HPLC) on a reverse phase C-18 column with UV detection to identify and quantify metformin in methanol extract of Myrmeleon sp. Several parameters of HPLC method were optimized in accordance with the high resolution of separation and accurate determination of metformin. Satisfied separation was obtained under gradient elution mode using aqueous methanol/water with gradual increase of methanol from 50-90% and the flow rate of 0.5 mL/min with detection wavelength of 233 nm. The method performed total separation for all compounds in less than 11 min. Spiking technique was chosen for metformin identification and quantitation. Metformin in extract Myrmeleon sp. was eluted at the retention time (t R ) of 4.095 min, similarly with the retention time of standard metformin of 4.092 min. The quantity of metformin in Myrmeleon sp. can be simply determined by comparing the additional area of standard metformin and area of metformin from extract Myrmeleon sp. The results confirmed that the methanol extract of Myrmeleon sp. contained the anti-diabetes compound of metformin of 0.58 mg/g Myrmeleon sp. larvae with acceptable coefficient variation of 5.56 %.

Despite the extensive clinical experience with the use of metformin worldwide, no formal doseranging study has been conducted because the current dosing strategy of metformin was determined empirically, rather than by an understanding of its dose-response relationship in patients with type 2 diabetes. The present study was designed to evaluate the correlation between serum metformin levels and glycemic control, insulin resistance and leptin levels in females newly diagnosed with type 2 diabetes. Sixty type 2 diabetic females were recruited for the study and were allocated into 3 groups; each receiving metformin 1000, 1500 and 2000 mg/day respectively for 3 months. Blood samples withdrawn from each patient at zero time and after 3 months is used to evaluate serum levels of HbA1c, glucose, leptin and insulin, in addition, the measurement of serum level of metformin in blood after 3 months by HPLC. The results demonstrated that all the treated groups with different doses of metformin showed significant improve in all parameters; the use of increased metformin doses was only correlated with plasma leptin levels in the highest dose. In conclusion, serum metformin levels are not good predictors for correlating improvement in clinical and biochemical parameters with increasing the dose in newly diagnosed non-insulin resistant females with type 2 diabetes.

Metformin has been known to the mankind for centuries. Metformin is the first line of drug of choice for the treatment of type 2 diabetes. Initial pharmaceutical therapy was for treating the frequent urinal problem which is a primary manifestation of diabetes. At the time of writing, metformin as an intervention is looked into for cardiovascular risks, polycystic ovary syndrome and breast cancer. Annual demand for Metformin is 23,000 Metric tons per annum. Nano Drug Delivery Systems (NDDS) has embarked into Metformin controlled and targeted delivery for breast cancer, pancreatic cancer and non-small cell lung cancer. Clinical estimation of the biological fluids by the FDA approved diagnostic laboratories use LC-MS/MS where blood samples estimate Metformin as an intact molecule easing the pressure of clinical trials result. Metformin was manufactured by a less known and low profile Aoron pharmaceutical, France not aware of the block buster drug hence the potential of technology development start-ups can neither be underwritten nor underestimated even when there is a very high rate (~90%) of failed biotech/biomed start-up ventures.
