Mammography has long been considered as the primary technique in breast cancer detection and assessment. Despite low specificity, mammography has been preferred over other contemporary techniques such as magnetic resonance imaging (MRI), computed tomography (CT) and ultrasonography (US) due to superior sensitivity and significant health economic benefits. The development of a new technique, a limited angle cone beam pseudo‐three‐dimensional tomosynthesis, digital breast tomosynthesis (DBT), has gained momentum. Several preliminary studies and ongoing trials are showing evidence of the benefits of DBT in improving lesion visibility, accuracy of cancer detection and observer performance. This raises the possibility of adoption of DBT in the breast cancer assessment clinic, wherein confirming or dismissing the presence of malignancy (at the potential site identified during screening) is of utmost importance. Identification of suspected malignancy in terms of lesion characteristics and location is also essential in assessment. In this literature review, we evaluate the role of DBT for use in breast cancer assessment and its future in biopsy.

Background: This study aimed to systematically review and to meta-analyse the accuracy of digital breast tomosynthesis (DBT) versus digital mammography (DM) in women with mammographically dense breasts in screening and diagnosis. Methods: Two independent reviewers identified screening or diagnostic studies reporting at least one of four outcomes (cancer detection rate-CDR, recall rate, sensitivity and specificity) for DBT and DM in women with mammographically dense breasts. Study quality was assessed using QUADAS-2. Meta-analysis of CDR and recall rate used a random effects model. Summary ROC curve summarized sensitivity and specificity. Results: Sixteen studies were included (five diagnostic; eleven screening). In diagnosis, DBT increased sensitivity (84%-90%) versus DM alone (69%-86%) but not specificity. DBT improved CDR versus DM alone (RR: 1.16, 95% CI 1. 02-1.31). In screening, DBT + DM increased CDR versus DM alone (RR: 1.33, 95% CI 1.20-1.47 for retrospective studies; RR: 1.52, 95% CI 1.08-2.11 for prospective studies). Recall rate was significantly reduced by DBT + DM in retrospective studies (RR: 0.72, 95% CI 0.64-0.80) but not in two prospective studies (RR: 1.12, 95% CI 0.76-1.63). Conclusion: In women with mammographically dense breasts, DBT+/−DM increased CDR significantly (versus DM) in screening and diagnosis. In diagnosis, DBT+/−DM increased sensitivity but not specificity. The effect of DBT + DM on recall rate in screening dense breasts varied between studies.

Purpose Although breast density is considered a strong risk factor of breast cancer, its quantitative assessment is difficult. To investigate a quantitative method of measuring breast density using dual‐energy mammographic imaging with central digital breast tomosynthesis in physically uniform and nonuniform phantoms. Material and methods The dual‐energy imaging unit used a tungsten anode and silver filter with 30 kVp for high‐energy images and 20 kVp for low‐energy images. Uniform glandular‐equivalent phantoms were used to calibrate a dual‐energy based decomposition algorithm. The first study used uniform breast phantoms which ranged in thicknesses from 20 to 70 mm, in 10‐mm increments, and which provided 30%, 50%, and 70% of breast density. The second study used uniform phantoms ranging from 10% to 90% of breast density. The third study used non‐uniform phantoms (at an average density of 50%) with a thickness which ranged from 20 to 90 mm, in 10‐mm increments. Results The root mean square error of breast density measurements was 2.64–3.34% for the uniform, variable thickness phantoms, 4.17% for the uniform, variable density phantoms, and 4.49% for the nonuniform, variable thickness phantoms. Conclusion The dual‐energy technique could be used to measure breast density with a margin of error of < 10% using digital breast tomosynthesis.
