Depression is Associated with Sexual Risk Among Men Who Have Sex with Men, but is Mediated by Cognitive Escape and Self-Efficacy

Abstract: Men who have sex with men (MSM) show high rates of HIV infection, and higher rates of depression than non-MSM. We examined the association between depression and sexual risk among "high risk" MSM. Evidence has been mixed regarding the link between depression and risky sex, although researchers have rarely considered the role of psychosocial vulnerabilities such as self-efficacy for sexual safety or "escape" coping styles. In a national sample (N = 1,540) of HIV-positive and HIV-negative MSM who reported unprot… Show more

“…Poor mental health, including depressed mood and low self-esteem, has been linked to sexual risk behavior in prior studies of adult MSM. [45][46][47] Finally, negative father-son relationship dynamics increased situational risk for some participants by predisposing them to form partnerships where they had limited power to negotiate safe sex. At its extreme, paternal rejection led to homelessness and exchanging unsafe sex in return for housing, a phenomenon that has been previously described in other gay youth.…”

“…In the relationship between perceived self-efficacy and health, the former, along with other components of SCT, such as outcome expectancies, has emerged as one of the most important predictors of a number of health-related behaviors in a diverse range of studies 12,13,14 . Perceived selfefficacy is directly related to the adoption and the promotion of healthy behaviors, e.g., prevention of unsafe sexual behavior 15,16 , getting regular physical exercise 17 , proper nutrition and weight control 18 , and smoking cessation 19 . It is also related to the positive and negative affects and strategies for coping with stress 14,20 .…”

“…24 Disclosure self-efficacy is a strong predictor of disclosure, 9 such that those with lower disclosure self-efficacy are more likely to avoid disclosure or inconsistently disclose to sexual partners. 25 The negative impact of depression on self-efficacy for safer sex has been demonstrated for MSM, 26,27 but it is not known how depression influences self-efficacy for disclosure.…”
