Dental trauma involving root fracture and periodontal ligament injury: a 10-year retrospective study

This study investigated the level of knowledge held by dentists about the possible treatment plan procedures for periodontal ligament injuries after dentoalveolar trauma. A 5-item self-applied questionnaire was prepared with questions referring to the professional profile of the interviewees and to the treatment plan they would propose for periodontal ligament injuries secondary to dentoalveolar trauma. The questionnaires were filled out by 693 dentists attending the 23 rd Annual Meeting of the Brazilian Society for Dental Research, and the data obtained were subjected to descriptive analysis. Either the chi-square test or Fisher's exact test was applied to assess associations among variables, at a 5% level of significance. The results revealed that dentists experienced difficulty in establishing a treatment plan for subluxation, and for extrusive, lateral and intrusive luxations. In general, holding a dental specialty degree had no influence on the knowledge about treatment plan procedures for the most severe injuries. It could be concluded that the dentists participating in this study, whether specialists or not, did not have sufficient knowledge to treat most of the periodontal ligament injuries resulting from dentoalveolar trauma adequately.

ObjectivesThe aim of this study was to evaluate cases of dental trauma treated at the specialized center of Pontifical Catholic University of Paraná, Curitiba, Brazil, during a period of 2 years.Material and MethodsA total of 647 patients were evaluated and treated between 2003 and 2005. Data obtained from each patient were tabulated and analyzed as to gender, age, etiology, time elapsed after the injury, diagnosis (type of trauma), and affected teeth.ResultsThe results revealed that male individuals aged 7 to 13 years presented the highest prevalence of injury, and falling was the main causal factor. In most cases, the time elapsed between the accident and the first care ranged from 4 to 24 h. A total of 1,747 teeth were affected, with higher incidence of concussion/subluxation and coronal fracture, followed by lateral luxation and avulsion. The permanent maxillary central incisors were the most commonly affected teeth.ConclusionThe frequency and causes of dentoalveolar trauma should be investigated for identification of risk groups, treatment demands and costs in order to allow for the establishment of effective preventive measures that can reduce the treatment duration and costs for both patients and oral health services.

Ö ÖZ ZE ET T Lateral luksasyon dişin travma sonucu ekseninden farklı bir yöne yer değiştirmesidir. Subluksasyon yer değiştirme dişte mobilite artışına ve perküsyon hassasiyetine neden olan bir travma tipidir. Yirmi yaşındaki erkek olgu, travmatik yaralanma şikâyetiyle kliniğe başvurdu. Ağız içi muayenesinde maksiller sağ santral dişte lateral luksasyon, maksiller sol santral dişte subluksasyon saptandı. Konik ışınlı bilgisayarlı tomografide, lateral luksasyon gösteren dişin kök ucundaki alveolar kemikte kırık görüldü. Maksiller sağ santral diş ve kırık kemik bölgesi parmak basıncıyla repoze edildi. Maksiller kesici ve kanin dişlerini içine alan yarı-rijit "splint" yerleştirildi. İki ay vitalitenin geri dönmesi ve geçici periapikal genişleme amacıyla beklendi, ancak dişler bu sürenin sonunda elektrikli pulpa testi ile nekroze olarak değerlendirildi. İki ay sonunda "splint" çıkarıldı ve orta keser dişlere kanal tedavileri yapıldı. On iki aylık izlemde, klinik ve radyografik olarak hem periapikal bölgede hem de periodontal ligamentte patoloji saptanmadı. Bu olgu sunumunda, lateral luksasyon ve subluksasyon yaralanmalarının tedavisi ve oluşabilecek komplikasyonların değerlendirilmesi amaçlanmıştır.A An na ah ht ta ar r K Ke el li im me el le er r: : Diş yaralanmaları; endodonti A AB BS ST TR RA AC CT T Lateral luxation is displacement of the tooth in a direction other than axially. Subluxation is an injury to the tooth periodonsium causing abnormal mobility but without displacement. A 20-year old male patient who suffered traumatic injury referred to the clinic. Clinical examination revealed that maxillary right central incisor was laterally luxated and maxillary left central incisor had a subluxation injury. Cone beam computerized images displayed alveolar bone fracture at the site of the root tip of the lateral luxated tooth. Maxillary right central incisor and fractured alveolar bone were repositioned by finger pressure and a semi-rigid splint that involves maxillary incisors and canines had placed. For the possibility of revascularization and reinnervation by way of transient apical breakdown, the splint was maintained for two months. During this time period, the teeth were diagnosed necrotic with an electrical pulp tester. The splint was removed and central incisors had root canal therapy. At 12-month recall, the teeth showed no periapical or periodontal pathosis radiographically and exhibited no signs or symptoms clinically. This case report presents and discusses the management of lateral luxation and subluxation injuries.
