Dental compensation for moderate Class III with vertical growth pattern by extraction of the lower second molars

Abstract: Dental compensation of moderate skeletal Class III with a tendency to an anterior open bite with vertical growth pattern by extracting the lower second molars, combined with Class III elastics, resulted in an anterior rotation of the occlusal plane and mandible. Eighteen of 20 patients achieved a physiological overjet and positive overbite. A prerequisite for this therapy is the presence of lower wisdom teeth; a potential side effect is elongation of the upper second molars.

“…Hwang et al 13,14 suggested that occlusal planes associated with an anterior open bite were frequently too flat, and so treatment goals should involve achieving a steeper curve of Spee. In addition, Jacobs et al 15 reported that this treatment goal should be achieved by a clockwise modification of maxillary position and a counterclockwise rotation of the mandible by intrusion of the posterior teeth. In the present case, OP-SN increased by 2.4° and an optimal rotation of the mandible occurred which decreased the lower facial height significantly.…”

“…An alternative approach for patients who reject surgery is represented by a compensatory orthodontic treatment associated with tooth extractions [ 9 , 10 ]. The most commonly extracted teeth in treatment with camouflage purposes are bicuspids, but molar and incisor extractions are also described in the literature [ 11 , 12 ].…”
