Background This study aimed to retrospectively evaluate the treatment efficacy of clear aligners for individuals with mild skeletal Class III malocclusion. Methods A total of 35 patients with non-surgical Class III malocclusion that was treated by clear aligner technology (CAT) underwent analysis using the peer assessment rating (PAR) index, digital model analysis, and X-ray cephalometry. The PAR index was evaluated using plaster casts made before treatment (T0) and at the end of treatment (T1). Using 3DSMax software, a digital model of STL(Standard Template Library) scanned by intraoral scanners at T0 and T1 was reconstructed. Subsequently, an analysis was conducted to compare the distal movement of molars, arch width, and vertical height in three dimensions. X-ray cephalometry analysis was employed to evaluate alterations in the anterior tooth axis and mandibular plane. The significance level was established at P < 0.05. Results Throughout the treatment, the PAR index demonstrated a significant reduction in sagittal variation during the T1 phase, decreasing from 1.85 ± 1.064 to 0.07 ± 0.267. Additionally, the presence of anterior crossbite decreased from 3.33 ± 1.109 to 0.00 ± 0.000, suggesting a complete correction of the anterior crossbite. Digital model analysis indicated that the distal movement of the mandibular molars ranged from 1.7 to 2.0 mm. Additionally, the width of the dental arch in the mandibular premolar area exhibited a significant increase from 32.33 mm to 33.76 mm post-treatment. The SNB (°) decreased from 82.16 ± 3.91 to 80.89 ± 2.41, while the ANB (°) significantly increased from − 1.25 ± 2.12 to -0.07 ± 2.48. The MP-SN plane angle remained relatively stable, changing from 31.50 ± 9.77 to 31.83 ± 5.79 in X-ray cephalometric analysis. Conclusions CAT can effectively address non-surgical Class III malocclusion. In the sagittal plane, CAT can achieve an average of 1.7–2.0 mm distal movement of mandibular molars and demonstrates effective control in both horizontal and vertical directions.

Introducción. Los alineadores dentales se han convertido en una alternativa en el tratamiento ortodóntico actual por sus beneficios estéticos y de confort para el paciente, sin embargo, es importante determinar la eficacia de estos para realizar los diferentes movimientos dentales. Objetivo. Analizar la eficacia de los alineadores dentales en los movimientos de extrusión dental en comparación con la ortodoncia convencional. Metodología. Se implementó una revisión bibliográfica y documentada de la literatura digital sobre la eficacia de los alienadores dentales en comparación con la ortodoncia convencional en el movimiento de extrusión mediante las siguientes palabras claves: dental aligners”, “traditional orthodontics”, “orthodontic brackets”, “extrusión movements” y effectiveness” y se seleccionaron 29 artículos. Resultados. los alineadores dentales presentan dificultades en la implementación de los movimientos de extrusión tanto de los incisivos superiores e inferiores como para la expansión de la estructura vestibular. Conclusión. Existe información dispersa sobre la eficacia y la previsibilidad en los movimientos extrusivos tanto con alineadores dentales como con la ortodoncia convencional. Predictability of Orthodontic Space Closure Using Invisalign Clear Aligners: A Retrospective Study

Introduction Clear aligners have demonstrated success in achieving complex tooth movements. However, orthodontists have faced challenges related to the predictability of clear aligners. This retrospective study aimed to assess the predictability of ClinCheck® accuracy in space closure before and after Invisalign® treatment and to identify factors associated with the need for refinement. Methods Patient records from one private clinic in Makkah, Saudi Arabia, were analyzed, and a sample of 55 adult patients who had spacing and underwent Invisalign treatment were included. Data on demographic and orthodontic variables were collected, and a chi-square test was conducted to examine the association between the requirement for refinement and demographic as well as clinical/orthodontic factors. Furthermore, the initial and final space measurements were compared using paired t-tests across various demographic and clinical/orthodontic variables. Results After completing the treatment, 70.9% (N=39) of the cases did not require any orthodontic refinement. The mean final space measurement was higher for males compared to females (0.7 mm and 0.4 mm, respectively), individuals who received treatment in the upper compared to lower arch (0.5 and 0.4 mm, respectively), those with moderate compared to mild spacing (0.5 and 0.1 mm, respectively), and those with class III compared to class I Angle classification (0.9 and 0.3 mm, respectively). Additionally, patients with severe spacing had a significantly higher probability of requiring refinement compared to patients with mild spacing (adjusted odds ratio = 20.9; p < 0.05). Conclusion The study emphasizes the significance of careful patient selection and treatment planning, suggesting that orthodontists should consider overcorrecting in space closure when using clear aligners, especially in cases with more significant spacing.
