Deep‐seated rectal/anal basaloid carcinoma: useful immunocytochemistry in rare squamous cell carcinoma variants

Abstract: The diagnosis of basaloid variant of SCC in a rectal location can be very difficult, both on account of the uncommon location and because of the low specificity of morphological aspects on EUS-FNA smears. The immunocytochemical technique, including a limited spectrum of keratins (CK7, CK20, CK34betae12, and p63) is necessary to avoid this diagnostic pitfall.

“…In addition, the remaining LBC materials left over from cytologic analysis can be used for immunocytochemistry; one of the most important advantages of using LBC is the ability to perform immunocytochemistry, which has been used on cervical LBC for biomarkers such as P16. Today, LBC with immunocytochemistry is extensively used for non-gynecological specimens to improve the diagnostic accuracy or to know the expressions of the hormone receptors [20,21,22,23]. For most types of cancer, a biopsy is the only way to make a diagnosis.…”

“…Immunohistochemistry may be useful for confirming the tissue origin of the tumor, which expresses CK20, CK7, CK34BE12 and p63. 10,11 The patient in question had the basaloid type, for which chemotherapy was contraindicated because of leukopenia, with lung metastases. She was being followed up in the palliative care outpatient department.…”

“…Because of its rarity, particularly in the posttransplantation setting, the optimal treatment strategy has not been established. [1][2][3][4][5] Options include an immunologic approach to maximize potential graft-versus-tumor effect through withdrawal of immunosuppression and/or donor leukocyte infusion, radiation therapy, chemotherapy, and total body electron therapy.…”

“…CK7 was focally positive in one case and negative in the second case. 5 To our knowledge, no cases of cutaneous metastases from rectal SCC have been reported. Several cases in the literature describe metastasis to the liver and lungs via the portal circulation.…”
