D8/17, an alloantigen found on B lymphocytes, has been reported to be elevated in patients susceptible to rheumatic fever and may be associated with autoimmune types of neuropsychiatric disorders. The pediatricautoimmune-neuropsychiatric-disorders-associated-with-streptococci model is a putative model of pathogenesis for a group of children whose symptoms of obsessive-compulsive disorder and Tourette's disorder (TD) are abrupt and may be triggered by an infection with group A streptococci. As a test of this model, we have examined D8/17 levels on the B cells of patients with TD and acute rheumatic fever (ARF) along with those on the B cells of normal controls by flow cytometry. We have utilized several different preparations of D8/17 antibody along with a variety of secondary antibodies but have been unable to show an association with an elevated percentage of D8/17-positive, CD19-positive B cells in either ARF or TD. We did find, however, that the percentages of CD19-positive B cells in ARF and TD patients were significantly elevated compared to those in normal controls. Group A streptococcal pharyngitis patients also had an elevated percentage of CD19 B cells, however. These studies failed to confirm the utility of determining the percentage of B cells expressing the D8/17 alloantigen in ARF patients or our sample of TD patients. In contrast, the percentage of CD19-positive B cells was significantly elevated in ARF and TD patients, as well as group A streptococcal pharyngitis patients, suggesting a role for inflammation and/or autoimmunity in the pathogenesis of these disorders. D8/17 is an alloantigen found on B lymphocytes and has been reported to be elevated in patients with acute rheumatic fever (ARF) (7,8,11). It is detected using a murine monoclonal antibody, derived by immunization with isolated B lymphocytes obtained from patients with rheumatic fever or rheumatic heart disease (4, 20), and has been reported to be expressed on elevated percentages of B cells in over 90% of rheumatic fever patients (11). This has led to the hypothesis that D8/17 may be a susceptibility marker for the development of rheumatic fever. D8/17 expression on B cells has also been reported to be elevated in patients with pediatric autoimmune neuropsychiatric disorders associated with streptococci (PANDAS) (17,19). The PANDAS model was proposed by Swedo et al. (19) for children whose symptoms of obsessive-compulsive disorder appear abruptly and are triggered by an infection with group A beta-hemolytic streptococci similar to that of Sydenham chorea, a manifestation of ARF. These authors reported that the D8/17 marker was expressed on elevated percentages of B cells in 85% of PANDAS patients and 89% of Sydenham chorea patients compared to 17% of healthy controls. Subsequently, Murphy et al. (17) indicated that patients with obsessive-compulsive disorder and/or Tourette's disorder (TD) had a mean percentage of D8/17-positive B cells of 22% but that in control subjects the mean percentage of D8/17-positive B cells was 9%. ResumoNesta revisão narrativa, o nosso objetivo foi descrever as síndromes neuropsiquiátricas pós-estreptocócicas e discuti-las à luz das evidências científicas atuais sobre os possíveis mecanismos patogenéticos envolvidos. Nos últimos anos, uma série de distúrbios do movimento, como tiques, distonia, parkinsonismo, e transtornos psiquiátricos, como o transtorno obsessivo-compulsivo (TOC) e o transtorno de hiperatividade com déficit de atenção (THDA), vem sendo considerada parte do espectro das manifestações pós-estreptocócicas. O termo PANDAS (acrônimo do inglês: pediatric autoimmune neuropsychiatric disorder associated with streptococcus) foi inclusive cunhado para descrever um subgrupo de pacientes com TOC e tiques que exibe flutuação clínica dos sintomas associada a infecção estreptocócica. Entretanto a análise crítica das evidências clinicolaboratoriais não apóia esse espectro ampliado das manifestações pós-estreptocócicas. Apenas na coréia de Sydenham há evidências consistentes de patogênese mediada por processo auto-imune pós-estreptocócico. AbstractIn this narrative review, our objective was to describe the poststreptococcal neuropsychiatric syndromes and to discuss the possible pathogenetic mechanisms involved in their clinical expression. Recently, several movement disorders, such as tics, dystonia, and parkinsonism, and psychiatric disorders like obsessive-compulsive disorder (OCD) and attention deficit hyperativity disorder (ADHD), are being considered part of a putative spectrum of post-streptococcal infection disorders. The term PANDAS (pediatric autoimmune neuropsychiatric disorder associated with streptococcus) has been introduced to describe a subset of patients with these diagnoses in which onset of symptoms or symptom exacerbations are related to streptococcal infection. However the critical analysis of clinical and laboratory data does not support the hypothesis of an extended spectrum of post-streptococcal neuropsychiatric disorders. Only for Sydenham chorea there is consistent evidence for a post-streptococcal autoimmune mediated pathogenesis.

INTRODUÇÃOO Transtorno Obsessivo-Compulsivo (TOC) é um transtorno que atinge cerca de 2,5% de indivíduos na população geral¹. A sintomatologia característica do TOC também se faz presente em outras situações, em especial nos demais Transtornos do espectro ObsessivoCompulsivo, como a Síndrome de Tourette (ST). Apesar da prevalência e dos altos custos anuais decorrentes dos transtornos citados, pouco se sabe acerca de sua etiologia. Aceita-se, atualmente, a ocorrência de herança multifatorial nos indivíduos portadores, ou seja, uma herança genética associada à resposta do indivíduo a estressores ambientais². Entretanto, ainda não foi identificada a carga genética específica para pacientes suscetíveis ao TOC e demais transtornos associados. Postula-se, portanto, a importância dos estressores ou fatores de risco para o desenvolvimento de tais patologias.Uma das mais recentes teorias, envolvendo fatores de risco associados ao TOC, correlaciona a ocorrência de infecção estreptocócica precedendo quadro, por vezes transitório, de TOC e Transtornos de Tique (como ST) em crianças. Mesmo em se tratando de uma constatação recente, aplicável apenas a um quadro específico³ (sintomas subitamente estabelecidos ou exacerbação dramática de sintomatologia pré-existente), a "hipótese imunológica" pode representar um avanço no entendimento da fisiopatologia do TOC, bem como no seu tratamento.Partindo do modelo já conhecido da Coréia de Sydenham pós-febre reumática, assim como do quadro clínico, das alterações imunes e de neuroimagem observadas no TOC, foi proposto um subgrupo de TOC em crianças denominado PANDAS (pediatric autoimmune neuropsychiatric disorders associated with streptococal infections).
