Current Insight of Collagen Biomatrix for Gingival Recession: An Evidence-Based Systematic Review

Collagen (Col) is a naturally available material and is widely used in the tissue engineering and medical field owing to its high biocompatibility and malleability. Promising results on the use of Col were observed in the periodontal application and many attempts have been carried out to inculcate Col for gingival recession (GR). Col is found to be an excellent provisional bioscaffold for the current treatment in GR. Therefore, the aim of this paper is to scrutinize an overview of the reported Col effect focusing on in vitro, in vivo, and clinical trials in GR application. A comprehensive literature search was performed using EBSCOhost, Science Direct, Springer Link, and Medline & Ovid databases to identify the potential articles on particular topics. The search query was accomplished based on the Boolean operators involving keywords such as (1) collagen OR scaffold OR hybrid scaffold OR biomaterial AND (2) gingiva recession OR tissue regeneration OR dental tissue OR healing mechanism OR gingiva. Only articles published from 2015 onwards were selected for further analysis. This review includes the physicochemical properties of Col scaffold and the outcome for GR. The comprehensive literature search retrieved a total of 3077 articles using the appropriate keywords. However, on the basis of the inclusion and exclusion criteria, only 15 articles were chosen for further review. The results from these articles indicated that Col promoted gingival tissue regeneration for GR healing. Therefore, this systematic review recapitulated that Col enhances regeneration of gingival tissue either through a slow or rapid process with no sign of cytotoxicity or adverse effect.

The most effective treatment for spinal tuberculosis was by eliminating the tuberculosis bacteria and replacing the infected bone with the bone graft to induce the healing process. This study aims to synthesize and characterize nanohydroxyapatite-gelatin-based injectable bone substitute (IBS) with addition of streptomycin. The IBS was synthesized by mixing nanohydroxyapatite and 20 w/v% gelatin with ratio of 40:60, 45:55, 50:50, 55:45, 60:40, 65:35, 70:30, and 75:25 ratio and streptomycin addition as antibiotic agent. The mixture was added by hydroxypropyl methylcellulose as suspending agent. FTIR test showed that there was a chemical reaction occurring in the mixture, between the gelatin and streptomycin. The result of injectability test showed that the highest injectability of the IBS sample was 98.64% with the setting time between 30 minutes and four hours after injection on the HA scaffold that represents the bone cavity and coat the pore scaffold. The cytotoxicity test result showed that the IBS samples were nontoxic towards BHK-21 fibroblast cells and human hepatocyte cells since the viability cell was more than 50% with significant difference (p-value<0.05). The acidity of the IBS was stable and it was sensitive towards Staphylococcus aureus with significantly difference (p-value<0.05). The streptomycin release test showed that the streptomycin could be released from the IBS-injected bone scaffold with release of 2.5% after 4 hours. All the results mentioned showed that IBS was suitable as a candidate to be used in spinal tuberculosis case.

Corneal disease is the 2nd biggest reason of blindness after cataract. Based on WHO data, it was predicted that 10 million people in the world would suffer from cornea-related sight deficiency. One of the solutions is by using wound dressing to cover the cornea so that it could heal effectively. Several studies were conducted to observe the use of collagen, hyaluronic acid, and PEO for wound dressing. To increase the properties of the membrane, chitosan coating was introduced. In this study, we observe the effect of chitosan coating on the electrospun membrane based on collagen, hyaluronic acid, and PEO combined with glutaraldehyde as a crosslinker. The result in FTIR test showed that the crosslinking process ended up in new bond formed among the materials used in this study. The chitosan coating could increase the transparency of the membrane as shown by the result of UV/Vis Spectrophotometry test but the result was not significantly different (p-value > 0.05). The morphology test using SEM showed that the pore diameter of the membrane decreased with the presence of chitosan coating (p-value < 0.05). Likewise, with an increase of chitosan coating concentration, the pores diameter also decreased. The cytotoxicity test showed that the membrane with several chitosan coating concentrations were non-toxic with cell viability more than 50 % (p-value < 0.05). The chitosan coating also had antibacterial effect as shown by the inhibition zone (p-value < 0.05), especially to Pseudomonas aeruginosa as gram positive bacteria. In conclusion, the presence of chitosan coating could improve the characteristics of the electrospun membrane coated with chitosan for wound dressing in corneal ulcers case.
