Cutaneous Protothecosis

Abstract: SUMMARY A case of human infection with Prototheca wickerhmii is described. The infection involved the skin and subcutaneous tissues of the forehead and scalp and the organism was demonstrated histologically and grown in culture. There is an early response to potassium iodide treatment but the final outcome is still uncertain.

“…In this patient, the clinical feature was cutaneous in nature with ulcerated papulo-pustular eruptions similar to earlier reported cases (2,7,(9)(10)(11)14,15). This may lead toa misdiagnosis of bacterial pyoderma or necrotizing vasculitis, therefore the differential diagnosis of algal should be excluded by laboratory examination from tissue specimens and exudates (13).…”

“…Davies et al (3,4) first described cutaneous Protothecosis caused by Prototheca segbwema in 1964. Thereafter, reports of more cases appeared in the literatures (2,7,(9)(10)(11)(13)(14)(15). It is believed that immunological defects, e. g. autoimmune diseases or the uses of immunosuppressive agents, often induce opportunistic infections (1,6,12).…”

“…However, none were effective other than surgical excision. Temporary improvement by potassium iodide had been documented, but it was later suggested that the lesion might have shown spontaneous regression (13,26). Systemic amphotericin B therapy with or without tetracycline were reported effective in mid 1970's (29,52,53).…”
